Jeong Hee Kim, Mi Yeul Hyun, Young Soon Kim, Jung Sil Kim, Sang Ok Nam, Man Sook Song, Jung Wha Lee, Na Ju Lee
J Korean Acad Nurs Adm 2010;16(1):26-36.   Published online March 31, 2010
DOI: https://doi.org/10.11111/jkana.2010.16.1.26
PURPOSE
The purpose of this study was to investigate the relationships among psycho-social well-being, and health perception and behavior in clinical nurses.
METHODS
The data were collected from 362 nurses employed at five general hospitals. A self-reported questionnaire was used to assess demographics and job characteristics, health perception and behavior, depression, and psycho-social stress. The data were analysed using Cronbach's alpha, descriptive statistics, t-test, and ANOVA.
RESULTS
While nurses perceived themselves to be in good health, represented by a high mean for health perceptions, actual health behavior was poor as the mean for health behavior was low. Forty two percents of the subjects were categorized as having major depression and high-stress levels. There were statistically significant differences in psycho-social well-being according to age, marital status, years of employment, and monthly salary. Also, there were significant differences in psycho-social well-being according to health perception, regular meals and exercise, sufficient sleep, job related injury and accident, and absences for ill.
CONCLUSION
In order to prevent depression and improve psycho-social well-being among clinical nurses, organizational support and health management needs to be focused on high-risk nurses. Furthermore, it is essential to consider the health perception and behavior of clinical nurses.

Predictors of the Clinical Competence in New Graduate Nurses
Youn Wha Shin, Haejung Lee, Yeonjung Lim
J Korean Acad Nurs Adm 2010;16(1):37-47.   Published online March 31, 2010
DOI: https://doi.org/10.11111/jkana.2010.16.1.37
PURPOSE
The purpose of this study was to identify the predictors of clinical competence in new graduate nurses.
METHODS
The subjects of this study were 238 nurses at 13 general hospitals who have had less than 12 months of nursing experience. The data were collected by structured questionnaire from August 5 to August 31 of 2009 and analyzed by the SPSS Win 12.0 program.
RESULTS
The total mean score for clinical competence was 181.05+/-15.17, critical thinking disposition was 94.65+/-8.12, and practice environment was 41.00+/-5.55. There were significant differences of clinical competence according to the GPA (t=-3.58, p<.001), the number of beds in the hospital (t=-3.22, p=.001), instruction by preceptor (t=-2.32, p=.021), and previous experience of clinical practice in the hospital (t=-2.21, p=.028). Additionally, critical thinking disposition and practice environment were positively correlated to clinical competence (r=.50, p<.001; r=.20, p=.002). In multivariate approach, predictors included in this study explained 43% of variance in clinical competence. Significant predictors of clinical competence were critical thinking disposition (beta=.50, p<.001), practice environment (beta=.14, p=.012), and working duration (beta=.13, p=.018).
CONCLUSIONS
Based on these findings, it is needed that providing supportive practice environment and developing curriculum for enhancing the critical thinking disposition to improve the clinical competence in new graduate nurses.

PURPOSE
The purpose of this study was to identify the effect of a mentoring program as a strategy for the retention of clinical nurses.
METHOD
Research subjects were 20 mentors, 20 mentees and 22 clinical nurses for a control group of mentees. A mentoring program was developed by an expert committee and applied to the experimental group for six months. Data were collected at three and six months after the application of the program from September, 2008 to March, 2009. Data were analyzed using chi-square-test, t-test, Repeated Measures ANOVA, One-way ANOVA in SPSS/Win 15.0 Program.
RESULTS
Job satisfaction, organizational commitment, empowerment, and carrier commitment in the mentees of the experimental group were significantly higher than the nurses of the control group. Intention of resignation in mentees of the experimental group were significantly lower than the nurses of the control group. Also, there were significant interaction between the groups and the times of measurements in all variables.
CONCLUSION
The mentoring program showed positive impact on the variables for maintenance of clinical nurses. Recommendation: Based on these findings, retention strategies using mentoring programs were recommended to reduce clinical nurses' resignation and improve the competency of clinical nurses that leads to the productivity of nursing organizations.

Development of a Critical Pathway of Barbiturate Coma Therapy in the Management for Severe Brain Damage
Jung Sook Kim
J Korean Acad Nurs Adm 2010;16(1):59-72.   Published online March 31, 2010
DOI: https://doi.org/10.11111/jkana.2010.16.1.59
PURPOSE
This study is a descriptive research to analyze prognostic factors of barbiturate coma therapy (BCT) for severe brain damage patients, to develop a critical pathway (CP) based on the results of analysis and to examine the effect of its clinical application.
METHOD
We analyzed medical records of 76 patients who received BCT for more than three days between January 1999 to July 2005. Based on the results of the analysis, we developed a CP and applied it to 12 people during August-December of 2005.
RESULT
By application of BCT CP, the mortality rate decreased from 31.6% to 16.7%. It was found that the period of staying at ICU and total period of hospitalization were shortened by 2.78 (13.9%) days and 16.43 (29.4%) days, respectively. The Glasgow coma scale of the recovery group by CP application was 9.03 (4.64) at 72 hours post of BCT and 14.28 (1.82) at discharge from hospital, and DRS was 6.62 (6.38) points.
CONCLUSION
By verifying clinical validity of the suggested CP, we believe that we have obtained visible effects standardizing the treatment pathway of BCT for brain damage patients.

Analysis on Military Hospital Nursing Records by NANDA, NIC, NOC System
Myung Ja Kim
J Korean Acad Nurs Adm 2010;16(1):73-85.   Published online March 31, 2010
DOI: https://doi.org/10.11111/jkana.2010.16.1.73
PURPOSE
This study was to construct a useful nursing language system on military nursing field.
METHOD
Military hospital nursing records were analyzed using NANDA(North American Nursing Diagnosis Association), NIC(Nursing Interventions Classification), and NOC(Nursing Outcomes Classification) systems. All kinds of nursing statements from 80 sets of orthopedics inpatient's records were deduced. All nursing statements were mapped to 167 NANDA diagnoses, 433 NIC interventions, and 260 NOC outcomes.
RESULT
14,744 nursing statements were extracted. Among the extracted nursing statements, 11.75% were linked with NANDA diagnosis, 83.62% were connected with NIC intervention, and 0.96% was tied to NOC outcome. 3.66% of nursing statements were not linked with NANDA-NIC-NOC system. In the nursing statements, 18 diagnoses of NANDA, 63 interventions of NIC, 8 outcomes of NOC were used.
CONCLUSIONS
The majority of those nursing statements focused on nursing intervention of the nursing process; few nursing plans or goals were found in nursing records. Therefore, it's difficult to make the nursing process network with the nursing statements. Documenting nursing records using a nursing process will contribute to strengthen nursing practice in patient care and to develop nursing as science. Continuous further researches related to nursing records are needed to provide basic data for developing nursing language system and nursing record system.

A Study on the Job Satisfaction and Organizational Commitment among Perioperative Nurses
Kesook Yoon
J Korean Acad Nurs Adm 2010;16(1):86-100.   Published online March 31, 2010
DOI: https://doi.org/10.11111/jkana.2010.16.1.86
PURPOSE
This study was done to examine the relationship of job satisfaction and organizational commitment of perioperative nurses.
METHOD
The subjects of this study were 500 perioperative nurses from 11 hospitals. The data were collected by self-reporting questionnaires from Sep. 19 to Sep. 27, 2009.
RESULTS
There was statistically significant relationship among the five variables. The analyses of covariance of these five variables revealed overall significant (p<.05). Stepwise linear multiple regression analyses were used to examine the influence of these five variables. Results indicated that the variables for verbal abuse (p<.01), workplace climate (p<.01), internal marketing (p<.001), and job transfer (p<.001) contributed significantly to the job satisfaction (adjusted R square=.426), while the verbal abuse (p<.01), internal marketing (p<.01), leadership style (p<.001) and workplace climate (p<.001) did to the organizational commitment (adjusted R square=.351). Canonical correlation analyses revealed that internal marketing and workplace climate contributed most significantly both to job satisfaction and organizational commitment.
CONCLUSION
This study found that all these five nursing managerial factors were important influential on both job satisfaction and organizational commitment of perioperative nurses. Addressing these factors with further research will surely improve the commitment of these nurses and ultimately lead to better perioperative nursing care.
