PURPOSE
The purpose of this study was to ascertain current status of nurses' perception, nursing performance, job stress, and burnout in relation to the Joint Commission International (JCI) hospital accreditation and to verify the relationships among these variables.
METHODS
This cross-sectional study was performed using questionnaires. Data were collected from 220 nurses who worked at one hospital from April 5 to May 4, 2013.
RESULTS
The scores for perception, nursing performance, job stress and burnout in relation to the JCI hospital accreditation were(on a five-point Likert scale) 3.23, 4.01, 3.56 and 3.40 respectively. A positive correlation was observed between perception and nursing performance. Burnout was negatively correlated with perception and nursing performance, and positively correlated with job stress.
CONCLUSION
These results indicate that developing positive perception and reduced burnout in relation to the JCI hospital accreditation can produce good nursing performance. These findings can be utilized to develop strategies for reducing job stress and burnout in relation to the JCI hospital accreditation.

Determinants of Registered Nurse Skill Mix & Staffing Level in Korea

PURPOSE
This study was done to identify determinants of registered nurse (RN) skill mix and staffing level focused on hospital characteristics.
METHODS
Data were obtained from health insurance claims data and hospital reporting system in the Health Insurance Review and Assessment Service (HIRA) for the year 2010. Data from 2,998 hospitals were analysed using t-test, ANOVA, Pearson correlation, and regression analysis.
RESULTS
The RN skill mix and staffing level were positively related to hospital size and the percentage of inpatients to total patients. RN skill mix and staffing level were statistically different across regions. Including nursing aides (NA), however, there was no difference in staffing levels across regions. Medically vulnerable regions, bed operation rate, and the number of patients per doctor were also related to RN skill mix and staffing level.
CONCLUSION
The statically significant determinants of RN skill mix and staffing level included hospital size, region, bed operation rate, percentage of inpatients, doctor-patient ratio. Further study needs to be done to investigate factors including RN supply and wages.

Adaptation Process of Mothers-in-law of Vietnamese Women Married to Korean Men

PURPOSE
This study was conducted to generate a grounded substantive theory of the adaptation process of mothers-in-law of Vietnamese women married to Korean husbands.
METHODS
Thirteen women who had Vietnamese daughters-in-law were interviewed. Data were transcribed verbatim and analyzed using a grounded theory method.
RESULTS
Eight categories with 19 sub-categories were extracted from 268 concepts. The identified phenomenon was 'overcoming differences' and the core category was 'trying one's best to live together with daughter-in-law'. The 9 categories were grouped into 3 stages for the adaptation process: encountering, struggling, and living together.
CONCLUSION
The results indicate that when individuals from different cultural and personal backgrounds have to live together there is a continuing negotiation process towards meeting each other's needs. Health professionals can assist this adaptation process by providing these women with insights into various ways of meeting each other's need while they are struggling.

A Phenomenological Study on Nurses' Experiences of Accidents in Patient Safety

PURPOSE
The purpose of this study was to describe nurses' experiences of accidents in patient safety.
METHODS
Data were collected from October 8, 2011 to January 31, 2012 through in-depth interviews with seven nurses who had worked on wards or in the ICU in a university hospital. Data were analyzed by applying Colaizzi's phenomenological methodology.
RESULTS
The following six categories were extracted: Fear of the patient's condition caused by the accident, Conflict in the accident report, Blame on others and circumstances, Feeling guilty and sorry as the patient's condition is improving, Being disappointed with the unfavorable atmosphere in dealing with the accident, After the accident, being sensitive in performing nursing duties and being faithful to the principles.
CONCLUSION
The results indicate that the organizational culture in the hospital related to accidents in patient safety is still closed and punitive, and such an atmosphere causes nurses to feel seriously hurt, but through this experience nurses are likely to mature as nursing professionals. Programs on prevention of accidents in patient safety and a system to guard against these accidents should be established. Also the organizational safety culture should be improved.

Effects of Resilience and Job Satisfaction on Organizational Commitment in Korean-American Registered Nurses

PURPOSE
The purpose of this study was to examine the effects of resilience and job satisfaction on organizational commitment in Korean-American registered nurses.
METHODS
A cross-sectional study was conducted using a structured questionnaire survey with 203 Korean-American registered nurses living in New York State and New Jersey State. Data were collected from May 8 to August 25, 2012. Collected data were analyzed using t-test, ANOVA, Scheffe test and hierarchical multiple regression.
RESULTS
The mean organizational commitment of Korean-American registered nurses was 3.34+/-0.59 out of a possible 5.00. The resilience and job satisfaction were significant variables predicting the level of organizational commitment among Korean-American registered nurses, accounting for 50% of the variability.
CONCLUSION
The results of the study indicate that it is necessary to identify factors influencing job satisfaction and develop programs to strengthen personal resilience in order to increase organizational commitment.

PURPOSE
The purpose of this research was to delineate the effects of organizational injustice on negative behaviors of employees with the mediating effects of trust.
METHODS
A survey was conducted among 550 nurses in 11 departments of 3 hospitals located in the Pohang area. Data were analyzed using frequency, descriptive statistics. correlation coefficients, regression analysis.
RESULTS
Procedural injustice (beta=-.48, p<.001) had the greatest negative effect on organizational trust, followed by interactional injustice(beta=-.15, p<.001) and distributive injustice(beta=-.14, p<.001). For supervisor trust, both procedural (beta=-.11, p=.006) and interactional (beta=-.63, p<.001) injustice had a negative effect. While supervisor trust (beta=-.28, p<.001) had greater negative effects on organizational negative behaviors of nurses compared to organizational trust (beta=-.21, p<.001), supervisor trust (beta=-.29, p<.001) had negative effects on personal negative behaviors but organizational trust had no significant effect on personal negative behaviors.
CONCLUSION
To reduce the negative organizational behaviors of nurses, an organizational effort is needed to reduce procedural injustice by introducing fair organizational management and to reduce interactional injustice through formal or informal communication channels between supervisors and nurses.

Effects of General Hospital Nurses' Work Environment on Job Embeddedness and Burnout

PURPOSE
The aim of this study was to investigate and identify work environment, job embeddedness, and burnout among general hospital nurses in Korea.
METHODS
The participants were 563 clinical nurses working in 13 general hospitals across the country. Data were analyzed using SPSS and Microsoft Excel programs.
RESULTS
Mean scores were 2.62 for nurses' work environment, 2.97 for job embeddedness, and 3.61 for burnout. Nurses' work environment showed a positive correlation with job embeddedness (r=.70, p<.001), but a negative correlation with burnout (r=-.49, p<.001). Subcategories of nurses' work environment that predicted job embeddedness included satisfaction and happiness, hospital support for the work environment, patient care environment, satisfaction with work schedule, manager leadership, supportive environment for nurses' work, and computer problems. Subcategories of nurses' work environment that predicted burnout included satisfaction and happiness, violence within ward, hospital support for work environment, and patient care environment.
CONCLUSION
Findings from this study indicate the need to evaluate and improve the work environment for nurses to increase job embeddedness and control burnout. Future studies should explore ways in which turnover intention can be decreased by changing nurses' work environment.

May 18 Democratic Uprising and Experiences of Nursing Activities in the Gwangju Area

PURPOSE
This study was conducted to define the experiences of nursing activities in Gwangju area during the May 18 (5 . 18) Democratic Uprising over 30 years ago.
METHODS
Data were collected in 2010 and 2011 through individual in-depth interviews. Ten nurses who worked in general hospitals in the Gwangju area during the 5 . 18 Democratic Uprising were interviewed using open-ended questions. Each interview lasted about 2~3 hours. Verbatim transcripts were analyzed using Van Kaam's method of phenomenological analysis.
RESULTS
Four categories emerged from the analysis: anxiety about the incident which was experienced for the first time; progress of the 5 . 18 events and relationship to participants; changes in nursing environment; retrospection and wishes related to the 5 . 18 incident.
CONCLUSION
This oral history study revealed more clearly of the experiences of nursing activities in the Gwangju area during the 5 . 18 Democratic Uprising. The data may provide a basis for writing history nursing in the community, and can be a basis for guidelines covering nursing care in crisis.

PURPOSE
The purpose of this study was to examine trends in number of nursing staff and skill mix.
METHODS
Nursing staff and skill mix were measured using the number of nursing staff including nurse aids and registered nurses per bed. Descriptive and panel data regression analyses were conducted using data on long-term care hospitals which included yearly series data from 2006 to 2010 for 119 hospitals.
RESULTS
The number of nursing staff per bed increased significantly but percentage of registered nurses decreased significantly from 2007 to 2010. The regression model explained this variation as much as 35% and 44%.
CONCLUSION
The results showed that in long-term care hospitals there were more nurse aids employed instead of registered nurses after the implemention of differentiated inpatient nursing fees. Thus clarifying the job descriptions for nurses and nurse aids is needed and appropriate hospital incentive policies should be implemented.

Issues and Challenges of Nurse Workforce Policy: A Critical Review and Implication

PURPOSE
In this paper issues and challenges of the workforce policy for nursing were explored and appropriate policy responses identified.
RESULTS
Many countries, including South Korea, are facing a shortage of nurses. In South Korea, the number of practicing registered nurses is about half the average for Organization for Economic Cooperation and Development (OECD) member countries. The shortage of nurses is not necessarily a shortage of individuals with nursing qualifications but also includes complex issues, such as lack of well-educated nurses, shortage of nurses willing to work, and geographical imbalances of nurses. The present nurse workforce policies are to increase number of nursing schools, to reduce the length of training, and to replace nurses with nursing assistants. However, the findings of many studies have shown that these attempts resulted in a worsening of patient outcomes and increasingly low quality of care.
CONCLUSION
The findings in this study indicate that nurse workforce policy should have a multi-faceted approach in order to address the many factors affecting nurse shortages.

Development and Long Term Evaluation of a Critical Pathway for the Management of Microvascular Decompression

PURPOSE
In order to provide a systematic and standardized treatment course for MVD patients, a critical pathway (CP) program was developed and the results of its long term application were analyzed.
METHODS
This was a methodological study. The CP was established and applied to 75 (step I) and 1,216 (step II). Another group of 56 with similar features was used as a control group.
RESULTS
The application of CP turned out to be useful in many regards: the rate of hearing loss was reduced from 1.8% to 0% (step I) and 0.5% (step II), and low cranial nerve palsy was reduced from 1.8% to 1.3% and 0.7%, respectively. The length of hospitalization decreased by 2.56 days (25.2%) for step I and 3.05 days (30.0%) for step II. Days of ICU stay were reduced by 7.9% and 1.8%. The total cost per patient was reduced by 14.8% (step I). The cost per day was increased by 13.7% and 52.4%. An increase in the patient satisfaction index was noted, as shown in the ICU information guide (p=.002).
CONCLUSION
The development and application of CP was found to improve the quality of medical treatment and the efficacy of hospital management in MVD patients. Well organized and efficient system and multidisciplinary teamwork are the key component of the successful application of CP.

An Example of Systematic Searching for Guidelines to Prevent Catheter-associated Urinary Tract Infections - Part I: Using the PubMed Database

PURPOSE
Effective literature searching is essential to support evidence-based nursing. The aim of this study was to present our recent systematic search experience to identify guidelines in PubMed for prevention of catheter-associated urinary tract infections.
METHODS
Five approaches to the literature search via PubMed were employed. The searches were restricted to items published from 1980 to 2010, for patients admitted to hospital, and in the English language. The search results were compared using the number of records and relevant items, and the sensitivity and precision of each search strategy.
RESULTS
The individual approaches retrieved 19-141 of records and 3-6 of relevant items. Sensitivity ranged from 37.5% to 75.0% with the highest values for simple searches and a search combining MeSH terms and free textwords with a methodological search filter. Precision varied from 4.3% to 21.7% and the highest precision was found for MeSH terms with limits feature.
CONCLUSION
The simple search in PubMed is an appropriate way for nurses in a busy clinical practice to search the literature for evidence. However, several approaches using MeSH terms, free textwords, limits feature or methodological search filters are also required to have more efficient and better informed search results.
