PURPOSE
The purpose of this study was to explore the degree of delay in waiting time, and the relationships of waiting time, patient satisfaction, and revisiting intention of outpatient in general hospitals.
METHODS
The data were collected from June 22 to July 4, 2009. A total of 536 outpatients who visited 21 clinics of a general hospital were subjected to evaluate the waiting time. The survey tools used were the Korea Health Industry Development Institutes (2008) tool for patient satisfaction and Reichheld & Sasser (1990) for revisiting intention. The data were analyzed by SAS version 9.1, descriptive statistics, t-test, ANOVA, and Pearson correlation coefficient.
RESULTS
The mean patient's waiting time was 28.3+/-30.7 min, the revealed mean score of patient's satisfaction was 2.92, and the revisiting intention showed was 4.56. The waiting time was negatively correlated with patient's satisfaction (r=-.10, p<.019). Patient's satisfaction was positively correlated with revisiting intention (r=-.51, p<.001).
CONCLUSION
Waiting time management is an important factor of increasing patient's satisfaction and revisiting intention in general hospitals. It is mandatory that reservation management systems take into account the patient's characteristics of visiting outpatient department in order to shorten the real waiting time.

The Impact of Nursing Professionalism on the Nursing Performance and Retention Intention among Psychiatric Mental Health Nurses

PURPOSE
This study aimed to investigate the impact of nursing professionalism on the nursing performance and retention intention among psychiatric mental health nurses.
METHODS
As a descriptive correlational study, this study sampled 206 psychiatric mental health nurses in six hospitals in Seoul and Gyeonggi area through convenience sampling. Data were collected from March 2 to 31, 2009 using a self-report questionnaire. The collected data were analyzed using SPSS WIN 16.0.
RESULTS
In the subscales of professionalism, the 'Sense of calling' had the highest mean score while the 'Professional organization' had the lowest mean score. A significant positive correlation was observed in nursing professionalism, nursing performance and retention intention. According to an analysis on the impact of each subscale of nursing professionalism on nursing performance and retention intention, the 'Sense of calling' and 'Autonomy' were the most significant predictor variable.
CONCLUSION
The results confirmed that the improvement of psychiatric mental health nurses' professionalism increases their nursing performance and retention intention and the 'Sense of calling' and 'Autonomy' are critical prediction factors. It is necessary to come up with a strategy which strengthens nursing professionalism in order to improve psychiatric mental health nurses' performance and retention intention.

Intensive Care Unit Nurse's Knowledge and Nursing Performance on Intensive Care Unit Syndrome

PURPOSE
This study was to investigate intensive care unit (ICU) nurse's knowledge and nursing performance on the intensive care unit syndrome. Method: A survey questionnaire was administered to a convenience sample of 123 nurses in a university hospital. Data were collected from August 1st to August 25th, 2009. Data were analyzed using SPSS/WIN 12.0.
RESULTS
The mean score for knowledge and nursing performance were66.96 and 74.97 respectively. According to the general characteristics, there were significant differences in knowledge depending on marital status, education level and career length and in nursing performance depending on their age, education level and career length. The correlation between knowledge and nursing performance was a significant positive one.
CONCLUSION
Comprehensive educational programs are needed to decrease accident rates related to ICU syndrome and to improve the health of ICU patients.

Factors Influencing Organizational Commitment among Hospital Nurses

PURPOSE
The purpose of this study is to examine the effective factors of organizational commitment among hospital nurses. Method: The subjects were 250 registered nurses working in 3 hospitals in Busan. Data were obtained by self-reported questionnaires from the 10th of June to 30th of June, 2009. Data were analyzed by frequency and percentage, t-test, ANOVA and Sheffe's test and stepwise multiple regression. Result: Mean of job satisfaction was 60.56+/-9.25 (range of scale: 20-100), internal marketing was 69.49 (range of scale: 25-125), job stress obtained 100.60+/-17.93 (range of scale: 45-180), and organizational commitment got 47.78+/-6.84 (range of scale: 15-75). The extent of the organizational commitment had differences according to the age (F=3.300, p=.039), educational level (F=3.21, p=.042), religion (F=3.30, p=.021), position (F=5.837, p=.003), and career length (F=2.642, p=.035). The influencing factors in organizational commitment of hospital nurses were job satisfaction, internal marketing, and age. R2=.503. The job satisfaction has the highest significant predictor of organizational commitment.
CONCLUSION
The influencing factors on organizational commitment were job satisfaction, internal marketing and age. Thus it is needed to improve the job satisfaction and internal marketing of hospitals' nurses and to provide care and support to younger nurses.

Influence of Clinical Nurse Specialists' Emotional Intelligence on Their Organizational Commitment and Turnover Intention

PURPOSE
The purpose of this study was to identify the influence of clinical nurse specialists' emotional intelligence on their organizational commitment and turnover intention.
METHODS
A descriptive study with second data analysis was conducted with permission. The original data was from the policy research on the status of clinical nurses with extended role hosted by the Hospital Nurses Association in 2007.
RESULTS
The mean score of emotional intelligence, organizational commitment, turnover intention were 58.66 (+/-6.63), 46.71 (+/-3.54), 12.87 (+/-3.98) respectively. Emotional intelligence had 40% positive influence on organizational commitment (F=50.58, p<.01, adj.R2=.16) and 24% negative influence on turnover intention (F=16.68, p<.01, adj.R2=.06).
CONCLUSION
The results recommend to develop a program that enhances clinical nurse specialists' emotional intelligence. The program could be utilized as an institutional retention strategy to increase clinical nurse specialists' organizational commitment and decrease their turnover intention.

The Effect of Learning Organization Construction and Learning Orientation on Organizational Effectiveness among Hospital Nurses

PURPOSE
This study conducted to identify the effect of learning organization construction and learning orientation on organizational effectiveness among hospital nurses. Method: Data was collected from convenient sample of 296 nurses who worked for the major hospitals in Seoul, Gyeonggi-do and Gangwoen-do. The self-reported questionnaire was used to assess the general characteristics, the level of the learning organization construction, learning orientation and organizational effectiveness. The data were analyzed using descriptive statistics, pearson's correlation coefficient and multiple regression. Result: The mean score of learning organization construction was 3.61(+/-.32), learning orientation got 3.26(+/-.39), and organizational effectiveness obtained 3.38(+/-.42). The learning organization construction affects of organizational effectiveness by 44.18% and learning orientation by 37.43%.
CONCLUSION
This finding indicates that learning organization construction and learning orientation affects the nurses' organizational effectiveness in hospital.

PURPOSE
The purpose of this study was to investigate the effects of organizational justice and dispositional affectivity on job satisfaction and intent to leave among nurses.
METHODS
The sample of this study consisted of 274 nurses from 2 general hospitals located in Incheon. Data were collected with self-administered questionnaires and were analyzed by hierarchical multiple regression.
RESULTS
Distributive and interactional justices had positive impacts on nurses' job satisfaction. Distributive, procedural and interactional justices had negative impacts on nurses' intent to leave. It was found out that positive affectivity significantly moderated the effect of interactional justice on job satisfaction while dispositional affectivity did not significantly moderate the effect of organizational justice on the intent to leave.
CONCLUSION
The results imply that hospital administrators should pay attention to the dispositional affectivity of nurses to increase their job satisfaction. Further, hospital needs to maintain organizational justice to reduce nurses' turnover.

An Analysis of Nursing Competency affecting on Job Satisfaction and Nursing Performance among Clinical Nurses

PURPOSE
The purpose of this study was to identify the effects of nursing competency of nurses on job satisfaction and nursing performance. Method: Using a structured questionnaire, data were collected from 368 nurses. Descriptive statistics, t-test, ANOVA, Scheffe test and pearson correlation coefficient with SAS package were used for data analysis.
RESULTS
The total mean score for nursing competency was 2.65, with scores for subcategories as follows: ethical competency 2.74, personal competency 2.65, esthetical competency 2.64, and scientific competency 2.61. The mean score of total job satisfaction was 3.18 on a 5 point scale, and nursing performance was 2.97 on a 4 point scale. Total nursing competency and total subcategories of nursing competency perceived by nurses were positively related to job satisfaction and nursing performance.
CONCLUSION
In conclusion, nursing competency of nurses influence job satisfaction and nursing performance. With these result, it is necessary to concentrate on improving nursing competency of nurses to increase job satisfaction and nursing performance.

An Evaluation of Effects on Hypertension and Diabetes Mellitus Management of a Community-Based Nursing Care Center Using Cost-Benefit Analysis

PURPOSE
This study was conducted to evaluate the economic efficiency of a community-based nursing care center to help policy makers determine whether or not to invest in similar facilities.
METHODS
The subjects were 101 elderly people over 65 years who participated in a health management program from February 1 to July 31, 2007. Direct cost was estimated with center operations cost, medical cost for out-patients and pharmacy cost. Indirect cost was measured by transportation cost. Direct benefit was calculated by saved medical cost for out-patients, saved pharmacy cost, saved transportation cost, and reducing hospital charges. Indirect benefit was estimated with prevention of severe complications. Economic efficiency was evaluated by cost-benefit ratio and net benefit.
RESULTS
Operating a community-based nursing care center was found to be cost-effective. Specifically, the cost of operating the center evaluated here was estimated at 135 million won while the benefit was estimated at 187 million won. Benefit-cost ratio was 1.38.
CONCLUSION
The Community-based nursing care center that was described here could be a useful health care delivery system for reducing medical expenditures.

Factors Associated with Work-Related Injuries of Nurses in Small and Medium Sized Hospitals

PURPOSE
This study was conducted to examine the factors associated with work related injuries of nurses in small and medium sized hospitals. Method: A cross-sectional survey was conducted with nurses in eight hospitals from October 2007 to January 2008. A questionnaire was designed to collect information on nurses' work related injuries, and individual and job related characteristics. The response rate was 65.1%. Data from 294 nurses were analyzed. Multiple logistic regression analysis was performed to determine factors associated with work related injuries. Result: Of the 294 nurses, 19.1% (n=56) responded as having at least one injury during their job performance. The logistic regression analysis revealed that the significant factors influencing work related injuries were job satisfaction, stress recognition, and hospital's location. Nurses with a higher job satisfaction were less likely to experience work related injuries (OR=0.58). Nurses with a higher stress recognition (OR=2.57) and those working at hospitals in metropolitan cities (OR=3.28) were more likely to experience work related injuries.
CONCLUSIONS
The result of this study indicated that a substantial proportion of nurses in small and medium sized hospitals had experienced injuries related to nursing job. Interventions to prevent work related injuries among nurses should take into account the job satisfaction, stress recognition, and hospital characteristics.

Career Ladder System Perceived by Nurses

PURPOSE
The purpose of this study was to investigate how nurses perceive the Career Ladder System(CLS).
METHODS
This study was a descriptive survey using questionnaires. Questionnaires were developed by an expert group. Participants included 161 nurses working in the Children's Hospital. The data were analyzed using factor analysis, Cronbach's alpha, descriptive statistics, chi-square-test and ANCOVA.
RESULTS
This study revealed that nurses who advanced in their career ladders have a significantly higher perception of participation in their professional activities and a general comprehension of the CLS more than the nurses who did not advance in their career ladders. However, nurses who advanced in their career ladders have a significantly lower perception of the expected outcome of the CLS more than nurses who did not.
CONCLUSIONS
This result showed that nurses who had experienced in clinical advancement and recognition were highly motivated with their professional activities via the CLS as well as they considered it to allow professional growth.

The Development of Nursing Standards for Coronary Artery Disease Patients Who Received Medical Treatment

PURPOSE
This study was conducted to develop nursing standards for medical patients with Coronary Artery Disease (CAD) patients.
METHODS
This study was a methodological study. The content of nursing standards for medical patients with CAD developed in this study was validated. The nursing standards of cardiovascular nursing developed by the American Nurses Association (2008), the nursing standards developed by the Korea Nurses Association (2003) and a clinical manual of patients with CAD developed by K teaching hospital were reviewed. Literature regarding CAD nursing standards was also reviewed. The basic contents of nursing standards for medical patients with CAD were selected by an expert group including two nursing faculties, a cardiovascular unit manager, and two cardiologists. A pilot study was conducted then in real clinical settings, in which includes cardiovascular outpatient clinic, cardiovascular inpatient units, and cardiac intensive care units to evaluate clinical suitability of the nursing standards.
RESULTS
The final version of the nursing standards for medical patients with CAD included 12 standards, 24 criteria, 38 indicators and 92 nursing activities.
CONCLUSION
The nursing standards developed in this study can be used in evaluating quality of nursing service and in educating nurses who are involved in patients with CAD.

The Effect of Head Nurse's Emotional Leadership on Nurse's Job Satisfaction & Organizational Commitment

PURPOSE
The purpose of this study was to identify the effect of head nurse's emotional leadership on nurse's job satisfaction & organizational commitment.
METHODS
The subjects of this study were 385 nurses from four general hospitals and one university hospital. SPSS WIN 14.0 was used for data analysis.
RESULTS
Stepwise multiple regression analyses were used to examine the influences of research variables. The variable which predict nurse's job satisfaction were head nurse's emotional leadership (F=76.027, p<.01, adjusted R square=.166). The variables which predict organizational commitment were emotional leadership (F=27.839, p<.01, adjusted R square=.066), marital status (F=20.928, p<.01 adjusted R square=.03), respectively.
CONCLUSIONS
As a result of this study, head nurse's emotional leadership was defined as a important influential on both job satisfaction and organizational commitment of nurses. Therefore, it is needed to develop education programs for activating head nurse's emotional leadership.

The Role of Clinical Research Nurses at Regional Clinical Trials Centers

PURPOSE
This study aimed to evaluate the performance of Clinical Research Nurses (CRNs) and the importance of their roles at the Regional Clinical Trial Centers (RCTCs). Method: A questionnaire focused on the role of CRNs was crafted by a researcher and the content validity was verified by a panel of experts on clinical research. The subjects of this study were 91 CRNs and Clinical Research Coordinators (CRCs), who were Korean registered nurses working at nine RCTCs. 77 subjects yielded valid data were analyzed using descriptive analysis, the Mann-Whitney U test, Spearman's rank order correlation coefficient, and Kruskal-Wallis test.
RESULTS
The performance of CRNs and the recognition in the importance of their roles were statistically significant different in age, education, CRN careers, positions, employment status and the phase of clinical trial. The role of direct caregiver was performed most often by CRNs. The role of coordinator of care and research (pre-study) was considered the most important role but performed the least frequent.
CONCLUSIONS
The role of CRNs can easily be differentiated from CRCs who are not registered nurses. The domains of CRNs should be clearly identified and established. Moreover, research should be carried out on CRN training programs to cultivate competence in CRNs.
