Influence of the Patient Safety Culture and Nursing Work Environment on Fall Prevention Activities of Hospital Nurses

Purpose
This study was conducted to identify the influence of patient safety culture and nursing work environment on fall prevention activities among hospital nurses. Methods: The participants were 177 nurses working at eight hospitals including clinics, general hospitals, and tertiary care hospitals located in B city. Data were collected using self-report questionnaires and analyzed using descriptive statistics, independent t-test, one-way ANOVA, Scheffé test. Pearson’s correlation coefficients, and hierarchical multiple regression with SPSS/WIN 26.0. Results: Fall prevention activities were positively correlated with nursing work environment (r=.25, p=.001) and patient safety culture (r=.49, p<.001). Fall prevention activities were found to be significantly affected by patient safety culture (β=.47, p<.001) and being a training hospital (β=.21, p=.006). Conclusion: The findings from this study suggest that patient safety culture is associated with fall prevention activities. In order to raise fall prevention activities, it is important to improve patient safety culture. Additionally, the health care organization should develop programs to raise fall prevention activities.

Factors Affecting the Degree of Harm from Fall Incidents in Hospitals

Purpose
This study aimed to examine the factors influencing the degree of harm caused by fall incidents in hospitals.
Methods
This cross-sectional descriptive study used secondary data from the “Korean Patient Safety Incident Report 2019” and was based on the International Classification for Patient Safety conceptual framework. We analyzed a total of 4,176 fall incidents between January 1 and December 31, 2019, in Korea. Multinomial logistic regression analyses were conducted to identify the factors that influence the degree of harm from these incidents.
Results
Among the fall incidents, 443 (10.6%) were sentinel events, 2,514 (60.2%) were adverse events, and 1,219 (29.2%) were near misses. The factors associated with sentinel events were old age (≥60 years), gender, patient diagnosis, medical department, early detection after falls, long-term care hospital, and reporters. Adverse events were significantly associated with diagnosis, medical department, nurses’ night shift time, large bed size, location (i.e., operating room, recovery room, and intensive care unit), and reporters.
Conclusion
Both patient and incident characteristics are significantly associated with the degree of harm in hospitals. Therefore, multiple factors should be considered to establish healthcare policies and thus prevent fall risks and minimize damage following falls in hospitals.

Development and Effects of Simulation Program for Fall Management

PURPOSE
This study was carried out to develop simulation scenarios for the management patient falls and to evaluate the effects of using the scenarios with student nurses.
METHODS
The research design was a quasi-experimental study using a methodology study. Study participants were 30 students who were in 4th year of nursing at one College of Nursing.
RESULTS
When comparing knowledge of falls before and after the simulation program, it was found that knowledge increased by 4.90 (from 24.60 pre-test to 29.50 post-test). For clinical performance of fall management, the score for assessment was 10.17 out of 16, for intervention 5.97 out of 10, and for evaluation 7.33 out of 8. The average score for reporting a fall to the doctor was 19.87 out of 30 based on SBAR. Prior to the implementation of the simulation program, the confidence of reporting to the physician was less than 5 in all four areas, but self-confidence improved by more than 6 points in all four areas after the program was implemented.
CONCLUSION
Findings indicate that results of fall management simulation practice can contribute to nursing students' knowledge of falls, as well as to nursing interventions and post-treatment following a patient fall.

Factors Influencing Nurses' Implementation of Evidence-based Fall Management in Geriatric Hospitals

PURPOSE
The purpose of this study was to identify factors that influence nurses' implementation of evidence-based fall management in geriatric hospitals.
METHODS
Data from the Evidence Based Practice Survey of Fall Management in Geriatric Hospitals were examined for this study. The participants were 248 nurses from geriatric hospitals. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation analysis, and stepwise regression.
RESULTS
Factors related to implementation of evidence-based fall management were belief in evidence-based practice with organizational culture, work experience, use of fall guideline, and experience of patient falls. These factors explained 56.7% of variance in implementation of evidence-based fall management. Among these factors, the significant predictors for implementation of evidence-based fall management were belief in evidence-based practice, organizational culture of evidence-based practice, work experience, use of fall guideline and experience of patient falls.
CONCLUSION
Results of this study suggest that it is necessary to develop a systematic program to enhance nurses' belief in evidence-based practice at the individual level. In addition, fostering organizational culture toward evidence-based practice and disseminating fall management guidelines at the organizational level can be important strategies to enhance evidence-based fall management in geriatric hospitals.

Effects of Health Belief on Fall Prevention Activities of Emergency Room Nurses

PURPOSE
The purpose of this study was to analyze factors affecting fall prevention activities of emergency room (ER) nurses based on their health belief factors (perceived susceptibility, perceived benefits, perceived severity, perceived barriers, and cues to action).
METHODS
The study design was a descriptive survey using questionnaires which were given to 127 emergency room nurses from two regional emergency medical centers, four local emergency medical centers, and two local emergency medical facilities. Data were analyzed using descriptive analysis, t-test, one-way ANOVA with LSD test, Pearson correlation, and multiple regressions.
RESULTS
ER nurses' fall prevention activities had a mean of 3.78+/-0.50. Eight individual characteristics and health belief factors accounted for 30.8% of the fall prevention activities. Fall prevention activities were found to be positively affected by emergency medical facilities, perceived benefits, and cues to action and negatively affected by factors, such as ER career and perceived severity.
CONCLUSION
The results indicate that it is necessary to formulate a plan for enhancing perceived benefits and cues to action to improve fall prevention activities. In addition, fall prevention activities should be encouraged for ER nurses who have worked in local medical institutions for less than 1 year or more than 5 years.

A Study on Self-leadership, Fall Attitude, and Nurses' Behavior to Prevent Patient Falls

PURPOSE
The purpose of this study was to identify self-leadership and attitude to patient falls and fall prevention behavior in nurses.
METHODS
The participants were 178 nurses from two general hospitals in Seoul. Data were collected from March 20 to May 10, 2012 using structured questionnaires which included a Self-leadership scale, Fall attitude scale, and Fall prevention scale. Data were analyzed using descriptive statistics, Pearson correlation and Hierarchial multiple regression with the SPSS/WIN 20.0 program.
RESULTS
The mean score for nurses' self-leadership was 3.48 (5 point scale). The average reported fall attitude was 3.59 (5 point scale) and fall prevention behavior was 3.26 (4 point scale). Self-leadership had a 18% explanatory power in fall attitude and a 5% explanatory power in fall prevention behavior. The factors which influenced fall prevention behavior were nurses' fall attitude (beta=.21, p=.009), fall prevention education (beta=.20, p=.007) and self-leadership (beta=.16, p=.047). All of the factors together had a 13.1% explanatory power.
CONCLUSION
The results of this study suggest that provision of regularly fall prevention education to nurses is required and should include a program to activate their positive attitude and self-leadership in order to improve fall prevention behavior in nurses.

Evaluation of a Fall Risk Assessment Tool to Establish Continuous Quality Improvement Process for Inpatients' Falls

PURPOSE
The aims of study were; (1) to evaluate the validity and sensitivity of a fall-risk assessment tool, and (2) to establish continuous quality improvement (CQI) methods to monitor the effective use of the risk assessment tool.
METHODS
A retrospective case-control cohort design was used. Analysis was conducted for 90 admissions as cases and 3,716 as controls during the 2006 and 2007 calendar years was conducted. Fallers were identified from the hospital's Accident Reporting System, and non-fallers were selected by randomized selection. Accuracy estimates, sensitivity analysis and logistic regression were used.
RESULTS
At the lower cutoff score of one, sensitivity, specificity, and positive and negative predictive values were 82.2%, 19.3%, 0.03%, and 96.9%, respectively. The area under the ROC was 0.60 implying poor prediction. Logistic regression analysis showed that five out of nine constitutional items; age, history of falls, gait problems, and confusion were significantly associated with falls. Based on these results, we suggested a tailored falls CQI process with specific indexes.
CONCLUSION
The fall-risk assessment tool was found to need considerable reviews for its validity and usage problems in practice. It is also necessary to develop protocols for use and identify strategies that reflect changes in patient conditions during hospital stay.
