Effects of Nurses' Positive Psychological Capital, Self-Leadership, and Relational Bonds on Organizational Commitment

Purpose
The purpose of this study was to investigate the association between nurses’ positive psychological capital, self-leadership, relational bonds, and organizational commitment, and to identify factors influencing organizational commitment.
Methods
The participants in this study were 199 nurses conveniently selected at one general hospital in Seoul. Data collection was conducted between January 1st and March 31st, 2019, and was analyzed using the SPSS/WIN 23.0 program.
Results
The mean score for positive psychological capital was 3.31±0.44, for self-leadership, 3.44±0.40, for relational bonds, 3.54±0.79, and for organizational commitment, 3.81±0.76. Positive psychological capital (r=.40, p<.001), self-leadership (r=.33, p<.001) and relational bonds (r=.51, p<.001) showed significant correlation with organizational commitment. Stepwise multiple regression analysis showed that positive psychological capital and relational bonds were included in the factors affecting the organizational commitment of clinical nurses. These variables explained 29.0% of organizational commitment.
Conclusion
Findings indicate that it is necessary to build effective strategies to enhance positive psychological capital and relational bonds for nurses. Such strategies will result in improved organizational commitment.

Relationships of Nurse-Nurse Collaboration and Nurse-Physician Collaboration with the Occurrence of Medical Errors

PURPOSE
The aim of this study was to examine degrees of nurse-nurse collaboration and nurse-physician collaboration, and investigate their relationships to the occurrence of medical errors.
METHODS
A cross-sectional questionnaire survey was conducted with 264 nurses in a university hospital. The questionnaire included fivecomponent nurse-nurse collaboration and three-component nurse-physician collaboration scales. Data were analyzed using independent t-tests, ANOVA, χ2 tests, and multiple logistic regression.
RESULTS
Mean score for nurse-nurse collaboration was 2.8 out of 4.0, and for nurse-physician collaboration, 3.4 out of 5.0. There were significant differences in the nurse-nurse and nurse-physician collaboration scores by nurses' preference to workplace and work unit. A significant difference was found in the nurse-nurse collaboration scores by job position. Fifty-seven (21.60%) nurses responded that they had experienced a medical error in the last six months. Logistic regression analysis revealed that nurse-physician collaboration was a significant factor associated with nurses' error experience. Nurses with higher scores for the nurse-physician relationship component were less likely to experience medical errors.
CONCLUSION
Findings from this study show that nurse-nurse and nurse-physician collaboration were moderate. Negative relationship between nurse-physician collaboration and the occurrence of medical error indicates that enhancing nurse-physician collaboration would contribute to improving patient safety.

Influences of Interpersonal Problems and Character of Nurses on Quality of Nursing Service

PURPOSE
In this descriptive correlation study, an investigation was done of the effects of interpersonal problems and character of nurses on the quality of nursing service.
METHODS
The participants were 154 clinical nurses working in hospitals located in C province and D city. Data were collected from February 1 to May 31, 2018. Data analysis was performed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and hierarchical regression.
RESULTS
Interpersonal problems correlated negatively with character and quality of nursing service. Character was positively correlated with quality of nursing service. Participants' age, marital status, education level, position, interpersonal problems and character explained 52.0% of variance in quality of nursing service.
CONCLUSION
The findings indicate that interpersonal problems and character are important factors to be considered in working to improve nursing service quality. Therefore, systematic programs to reduce interpersonal problems and enhance nurses' character level should be developed.

Effects of Relational Bonds and Professional Authority in the Organizational Commitment of Intensive Care Unit Nurses

PURPOSE
The aim of this study was to define the effects of relational bonds and professional authority in organizational commitment of Intensive Care Unit (ICU) nurses.
METHODS
A questionnaire was distributed to 200 ICU nurses working in 3 tertiary hospitals. The survey content included the Organizational Commitment Questionnaire (OCQ), Relational Bond Questionnaire (RBQ) and Schutzenhofer Professional Nurse Autonomy Scale (SPNA).
RESULTS
The score for OCQ was 2.59, for RBQ, 3.23, and for SPNA, 2.93. Organizational commitment was related to relational bonds (β=0.45; p<.001; SE=0.08). These factors accounted for 21.6% of the variability in organizational commitment.
CONCLUSION
Results indicate that ICU nurses' organizational commitment could be improved through increasing relational bonds and retaining experienced nurses which suggests a need to develop work systems designed to increase and maintain high relational bonds in ICU nurses.

Influences of Customer Orientation, Emotional Labor, Unit Manager-nurse Exchange and Relational Bonds on Nurses' Turnover Intension

PURPOSE
The study was done to investigate nurses' customer orientation, emotional labor, unit manager-nurse exchange and relational bonds and to identify the factors affecting nurses' turnover intentions.
METHODS
A cross-sectional survey was conducted using self-report questionnaires. The participants in this study were 276 nurses in tertiary hospital located in a metropolitan city, Korea. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and stepwise multiple regression with the SPSS program 22.0.
RESULTS
The mean scores were 3.74±0.90 (out of 5) for turnover intention, 5.31±0.87 (out of 7) for customer orientation, 4.82±0.96 (out of 7) for emotional labor, 3.58±0.61 (out of 7) for relational bonds, and 3.18±0.57 (out of 5) for unit manager-nurse exchange. Factors influencing turnover intention were 'emotional labor' (β=.39, p<.001), 'unit manager-nurse exchange' (β=-.22, p<.001), 'financial bonds' (β=-.19, p<.001), 'perceived economic status' (β=-.15 p=.003) and 'career' (β=.14, p=.005). These factors explained 34.9% of the variance in turnover intension (F=30.46, p<.001).
CONCLUSION
Findings indicate that to reduce nurses' turnover intention in the hospital, there is a need to improve unit manager-nurse exchange and to manage strategies lowering nurses' emotional labor.

Effects of Korean Enneagram Program on the Ego-identity, Interpersonal Relationship, and Self-leadership of Nursing College Students

PURPOSE
This study was done to identify the effects of the Korean Enneagram program on college nursing students' ego-identity, interpersonal relationships and self-leadership.
METHODS
A nonequivalent control group pre-posttest design was used. Twenty-nine students were in the experimental group receiving the Korean Enneagram program, and 28 were in the control group with no treatment.
RESULTS
'The experimental group will show higher scores for ego-identity (t=3.97, p<.001), interpersonal relationship (t=3.26, p=.002) and self-leadership (t=2.91, p=.007) compared to the control group 8 weeks after the program finished' was supported by significant differences in the scores between the two groups.
CONCLUSION
The findings indicate that the Korean Enneagram Program has a significant effect on improving ego-identity, interpersonal skills and self-leadership in nursing college students and the program can be used as a tool to promote the ego-identity, interpersonal skills and self-leadership of nursing students.

PURPOSE
The purpose was to compare operating room (OR) nurses and general ward nurses on their communication competence and interpersonal relationship ability within the medical team.
METHODS
Participants were 276 nurses (OR 122, ward 154) working in one of 4 university hospitals located in I and K areas. Data were collected using a questionnaire and data collection was done in June, 2013. Comparisons between OR nurses and general ward nurses on communication competence and interpersonal relationship ability were analyzed using ANCOVA.
RESULTS
There were statistically significant differences between the two groups in communication competence (F=11.96, p=.001) with average score for OR nurses at 3.37 points and ward nurses at 3.53 points. There were statistically significant differences between the two groups in interpersonal relationship ability (F=15.51, p<.001) with the average score for OR nurses at 3.30 points and ward nurses at 3.50 points.
CONCLUSION
The results indicate a need to develop a variety of programs to promote communication competence in OR nurses and to enhance the openness of interpersonal relationship ability in human resource management.

Role Transition of Senior Year Nursing Students: Analysis of Predictors for Role Transition

PURPOSE
The purpose of this study was to identify the perception of role transition from a student nurse to a registered nurse among senior year nursing students and to examine factors affecting their role transition.
METHODS
A descriptive survey with convenience sampling was conducted in four nursing colleges in Seoul, South Korea. Data were collected using a self-administrated questionnaire. Four instruments including role transition, self-esteem, interpersonal relationships and anxiety during clinical practicum were used and the students'demographics were also collected. A multiple logistic regression was used to identify predictors for the role transition.
RESULTS
A total of 233 nursing students were surveyed and final analysis was conducted utilizing 226 participants. Mean point of perceived role transition (5 point scale) was 3.34+/-0.44. In a multiple regression model, high self-esteem, good interpersonal relationships, low anxiety during clinical practicum, and high satisfaction in college life were significant predictors of a smooth transition (Adj. R2=.32, F=22.28, p<.001).
CONCLUSION
The findings suggest that role transition from a student nurse to a registered nurse is facilitated through the establishment of programs to improve self-esteem and interpersonal relationships and to reduce anxiety during clinical practicum.

PURPOSE
This study was done to propose an improvement in the Nursing Fee Differentiation Policy to alleviate polarization of nursing staffing level among hospitals and to rectify the confusion of legally mandated standards between the Korean Medical Law and National Health Insurance Act.
METHODS
The policy regulation was reconstructed related to nurse staffing standards and nurse-to-patients ratios. Data on nurse staffing grades were obtained from database of the Health Insurance Review & Assessment Service (HIRA) for the third quarter of 2010 for 44 tertiary hospitals, 274 general hospitals, and 1,262 hospitals. A break-even analysis was used to estimate financial burden of the revised policy improvement proposal. An industrial engineering method was used to calculate Nurse-to-Patients ratios per shift.
RESULTS
Twelve tertiary hospitals were downgraded. 74 general hospitals and 102 hospitals were upgraded after application of the regulation. Finances for total hospitalization expenditures changed from -3.55% to +3.14%.
CONCLUSION
The results indicate that the proposed policy would decrease polarization between tertiary hospitals and small hospitals, and would not put a major strain on the finances of the Korean National Health Insurance. Therefore, it is suggested that government stake-holders and many interest groups consider this policy proposal and build a consensus.

Interpersonal Relations, Hope, Professional Self-concept and Turnover Intention according to Adult Attachment Styles in Early Stage Nurses

PURPOSE
This study was done to examine interpersonal relations, hope, professional self-concept and turnover intention according to adult attachment styles in early stage nurses.
METHODS
A survey using a structured questionnaire was conducted with 245 nurses who work in one of six general hospitals. Additionally, a qualitative method was used to obtain data on experiences in interpersonal relations. Data were analyzed using SPSS/WINdows 14.0 and qualitative data from open question in the survey questionnaire were analyzed using content analysis.
RESULTS
There was a significant difference in interpersonal relations, hope, and professional self-concept according to adult attachment styles. In the interpersonal relation of early stage nurses, 8 themes were identified, of which the most frequent were: a comfortable relationship with my co-workers (43%) and stress from the relationship (34%).
CONCLUSION
Findings from this study should be useful in the development of programs to promote adult attachment and professional self-concept for early stage nurses.

PURPOSE
The purposes of this study were to analyze the mediating effects of empowerment, job stress, organizational commitment and relation-oriented nursing organization culture on turnover intention of clinical nurses.
METHODS
Participants selected for the final analysis were 382 nurses working in 4 general hospitals. Data were analyzed through descriptive statistics Pearson correlation analysis (SPSS/WIN 17.0), and Path analysis (AMOS 18.0).
RESULTS
The study results are as follows: relation-oriented nursing organizational culture had no direct effect but had an indirect effect on nurses' turnover intention through empowerment, job stress, and organizational commitment, while job stress and organizational commitment had direct effects on turnover intention. The results also showed that empowerment had strong effect on organizational commitment, but job stress had very weak effect. Empowerment had an effect on turnover intention.
CONCLUSION
These results indicate that establishment of relation-oriented nursing organization culture is imperative if clinical nurses' turnover intention is to be decreased.

Structural Factors Influencing the Quality Management Activities in Nursing Homes

PURPOSE
Nursing home quality indicators have been focused widely on result outcomes, not for the environment in that quality of service are delivered. This study aimed to examine structural factors influencing quality management activities in nursing homes.
METHOD
Sample was 170 nursing homes responded to the survey questionnaire which was distributed to the 543 nursing homes nation-wide. Data were collected on structural characteristics, types of services, and quality management activities. Data were analyzed with the descriptive statistics, Pearson correlations, and multiple regression.
RESULT
Most of the nursing homes were operated as free of charge by the social welfare ownership. Average number of residents was 52.1 with severe and mild dementia and bedridden status. In terms of quality management activities, 34% of the sample had CQI committee that focused their activities on services delivery process, performance appraisal, record keeping regularly. 30.6% of quality management activities were accounted for by the number of residents with dementia, the ratio of RN to residents, rehabilitation services, and social wefare services in nursing homes.
CONCLUSION
We recommend that more comprehensive quality management activities should be developed as process quality indicators in conjunction with the outcome indicators.
