PURPOSE
This study was done to identify trends in leadership-related research by reviewing studies on hospital nurses in South Korea.
METHODS
The research was conducted from April 2 to 14, 2018 and search databases were RISS, KISS, DBpia, KM base, NAL, and NDSL. Search terms were ‘leadership’ and ‘nursing’ in the Korean language, and total 141 papers were selected.
RESULTS
Analysis of the research on leadership showed that 96.4% of research methods were quantitative research, and 60.3% were predictive researches. For study settings, 46.2% were general hospitals, 96.5% were staff nurses. In the study of leadership variables, 60.3% were independent variables and recent leadership was used as a mediating variable (4.3%) and a moderating variable (2.8%). Among the leadership types, self-leadership (31.2%) was the most common, and the main variable related to leadership was job satisfaction (25.8%).
CONCLUSION
These results provide useful data for deriving new research ideas about nursing leadership. Furthermore, this study has significance for providing the evidence for developing new leadership programs and policies in nursing organizations.

리더십이란, 리더가 조직의 목표를 달성하고자 개인이나 집단의 행위에 영향력을 행사하는 것으로[1], 사람을 변화시켜 새롭게 하며 열정을 북돋아주고 영감을 주는 행위들을 의미한다[2]. 그러므로 집단 및 조직을 이루며 생활하는 인간은 조직이 존재하는 한 조직의 목표를 성취시키기 위한 리더의 존재가 필요하며[3], 리더에게 있어 리더십은 절대적으로 필요한 역량이기에, 리더십은 조직의 인적자원관리, 조직성과와 관련된 분야에서는 늘 달성해야 하는 목표이자 이상이다[4].
현대의 간호사들은 대상자가 건강과 안녕을 성취하도록 돕기 위한 돌봄 제공자로서의 역할 뿐 아니라 속한 조직에서의 다양한 관리활동 및 관련 역량을 요구받고 있으며[5, 6], 특히 21세기 급변하는 의료환경의 변화는 간호사로 하여금 더 많은 리더십 역량과 지식을 갖추도록 권고하고 있다[7]. 세계보건기구인 World Health Organization (WHO)가 2015년에 발표한 지속가능개발목표(Substainable Development Goal, SDG)는 모든 인간을 돌봄의 대상으로 하는 간호사의 활동영역이 보다 확대됨을 재확인시킨 바 있다[8]. 이처럼 간호사가 다양한 영역에서 기대되는 역할을 충분히 해내기 위해서는 먼저 리더십 역량을 갖출 것이 요구되기에 간호 분야에서 리더십의 가치가 더욱 커지고 있다[7].
간호 분야에서의 리더십이란 구성원을 이끌어 가고, 문제 상황을 적절히 해결하며, 구성원의 울타리가 되어줄 수 있는 기술이자 능력으로[9], 리더십이 있는 리더가 이끄는 간호조직은 긍정적인 조직문화를 형성하고 간호사의 직무만족을 상승시키며 이직율을 감소시키는 등 다양한 조직성과를 향상시키는 것으로 알려져 있다[10, 11]. 그렇지만 리더십의 단어 명명이나 소수의 리더자가 주로 주체가 되는 특성 때문에, 일반적으로 조직 구조상 직급이 높은 사람들만이 리더십을 발휘해야 하는 것으로 간주되어 상대적으로 직급이 낮은 일반간호사는 리더십에 대한 지각이 부족하고 무관심한 경향이 있다[9]. 그러나 현시대의 리더는 소수의 관리자만을 의미하지 않으며, 모든 간호사들 스스로가 리더로서[12] 자신의 능력을 발휘할 것을 요구하고 있다[13]. 간호사들 대부분은 단독으로 업무를 수행하기 보다 집단 또는 부서단위로 근무하고 있으므로 간호업무를 수행함에 있어 독자적인 간호업무 외에 여러 단위의 구성원과 조화를 이루어 업무를 수행할 필요가 있다. 그러므로 간호사에게 리더십은 자연스럽게 요구된다고 할 수 있으며[6] 간호의 모든 단계에서 리더십을 발휘하기 위해서는 교육과 훈련 등을 통한 리더십 역량 개발이 필요하다[14].
이렇듯 임상현장에서 간호사의 리더십이 필요하다는 인식이 강조되면서 한국간호교육평가원에서는 간호교육 프로그램에서 성취해야 할 12가지 학습 성과 중 두 가지를 리더십과 관련된 학습 성과로 선정하여, 학생 때부터 간호 리더십의 원리를 이해하고 이를 발휘할 수 있는 역량을 갖춘 학생이 배출될 수 있도록 리더십 관련 평가기준을 제시한 바 있다[15]. 이는 임상간호현장에서 요구되는 역량 중 하나가 리더십임을 입증하는 것이다.
우리나라 간호학 분야의 리더십이 연구되기 시작한 것은 1970년대이며[16], 초창기 간호 분야의 리더십 연구는 간호행정가 또는 수간호사 등 병원의 간호관리자를 대상으로 이루어지는 연구[16, 17]들이 대부분이었다. 이후 점차 일반간호사 및 간호대학생 등을 대상자로 하는 간호 리더십 연구[9, 18, 19]로 확대되었으나 간호 리더십에 대한 애매모호한 개념들, 본질적인 요소들과 그렇지 않은 것들에 대한 혼재의 문제들이 있어 이를 명료하게 정의하기 위한 개념분석 연구[13]가 최근에서야 이루어진 실정이다. 간호 리더십과 관련하여 체계적으로 문헌을 분석한 연구는 2006년에 수행된 간호조직에서의 리더십 분석연구[20]가 있으며, 이후 간호사의 셀프리더십에 관한 문헌분석[21], 국내 임상간호사의 셀프리더십 관련 변인에 대한 체계적 문헌 고찰 및 메타분석[22], 그리고 국내외 간호사들에게 실시된 리더십 중재 프로그램의 효과에 대한 메타분석 연구[23] 등이 수행되었다. 국외 문헌에서는 2017년까지 이루어진 간호 리더십 연구의 체계적 문헌고찰을 통해 리더십과 간호 인력성과 및 조직성과 사이의 충분한 근거를 제시한 바 있으나 [11] 국내에서는 2006년 이후[20] 간호 리더십 연구의 최신 경향을 반영한 연구는 찾아보기 힘든 실정이다.
그러나 최근 간호의 활동영역이 보다 확대 되어가고[24] 간호사가 수행하는 리더십에 대한 중요성 또한 강조되어 가고 있는[7] 현 상황에서 특정 리더십 변수에 대한 새로운 연구도 지속적으로 이루어져야 하지만, 현재까지 축적된 간호 리더십 연구에 대한 전반적인 경향을 되돌아보고 이를 토대로 앞으로 나아가야 할 간호 리더십 연구의 미래를 조망해 볼 필요가 있다. 특히 임상간호사 대부분이 속해있는 병원에서[25] 조직성과의 부분과 간호 리더십의 관계를 생각한다면 실제적으로 활용 가능한 리더십 관련 중재 개발 및 추후 리더십 연구방향 설정 시 사용될 수 있도록, 현재까지 진행된 국내 병원 간호사 대상 리더십 연구의 동향을 확인해 볼 필요가 있다. 이에, 본 연구에서는 국내 병원 간호사를 대상으로 이루어진 선행 리더십 연구를 고찰하고 관련 특성들을 분석해봄으로서 현재 국내 간호 리더십 연구의 흐름을 파악하고 불필요한 반복연구가 시행되지 않도록 안내하여 향후 국내 병원 간호 리더십 관련 연구 및 중재 개발 시 근거로 활용될 수 있는 기초자료를 제공하고자 한다.
본 연구의 목적은 국내 병원 간호사를 대상으로 한 간호 리더십 연구의 동향을 파악하기 위함으로 구체적인 목적은 다음과 같다.
• 간호 리더십 연구의 일반적인 특성을 조사한다.
• 연구된 간호 리더십의 종류를 파악한다.
• 간호 리더십 연구에서 리더십 변수의 특성을 조사한다.
• 간호 리더십 연구의 관련 변수 종류와 특성을 조사한다.
• 리더십 연구에서 리더십과 관련 변수들 간의 통계적 유의성을 조사한다.
본 연구는 국내 병원 간호사의 리더십 관련 연구의 동향을 분석한 서술적 조사연구이다.
본 연구의 조사 대상이 되는 논문의 구체적인 선정기준은 국내 병원에서 근무하는 간호사를 대상으로 한 연구로서 리더십이 대상연구의 주요 연구 변수인 연구, 학술지에 출판된 논문이다. 연구대상 논문의 배제기준은 연구대상자가 병원에서 근무하는 간호사가 아니거나, 리더십이 대상연구의 주요 변수가 아닌 경우, 학위논문, 문헌 분석 연구, 학술대회 발표 초록이나 proceeding, 원문을 이용할 수 없거나 국어 또는 영어 이외의 언어로 출판된 연구이다.
본 연구는 검색어 선정, 검색 데이터베이스 선정, 검색전략 수립, 문헌검색 수행, 분석대상 문헌 선정의 순서로 진행되었다. 자료검색은 2018년 4월 2일부터 4월 14일까지 이루어졌으며, 간호사를 대상으로 한 국내의 리더십 연구를 수집하였고 검색 기간은 제한하지 않았다. 검색 데이터베이스는 학술연구 정보서비스(Research Information Service System, RISS), 한국학술정보(Koreanstudies Information Service System, KISS), 누리미디어(DBpia), 한국의학논문데이타베이스(Korean Medical Database, KM base), 국회도서관(National Assembly Library, Nanet), 국가과학기술정보센터(National Digital Science Library, NDSL)을 이용하였다. 검색어는 국문으로 [“리더십” OR “리더쉽” OR “지도성” OR “리더”] AND [“간호”]로 선정하였다. 이와 같은 방법으로 검색된 문헌은 3,801편이었으며, 추출된 논문은 문헌 관리 프로그램(EndNote X8)을 이용하여 중복자료 2,929편을 제거하였다. 이후 872편의 문헌에 대하여 선정기준에 따라 논문의 제목과 초록을 확인하여 제외기준에 해당하는 문헌 730편을 제외하여 142편의 관련논문을 1차로 선택하였으며, 그 후 논문의 원본을 확보하여 검토하는 작업을 수행하였다. 자료를 선정하고 추출하는 전 과정은 2명의 연구자가 독립적으로 수행하였으며 두 연구자가 합의를 이루지 못할 경우 전체 연구팀이 개입하여 원문을 함께 검토하였다. 최종까지 원문 확보가 불가하였던 1편을 제외한 141편의 논문을 최종적으로 분석 논문으로 선정하였다(Figure 1).

본 연구에서 사용된 논문들의 분석틀은 국내에서 수행된 간호학분야의 선행 논문분석 및 동향연구들[20, 26, 27, 28]을 참고하여 본 연구의 목적에 적합하도록 본 연구팀이 개발한 것을 사용하였다. 분석틀의 주요내용은 연도, 제목, 저자, 연구자 수, 제 1저자 직종, 연구모형, 연구가설, IRB 승인여부, 표본 수 산출기준 제시, 효과크기 제시, 표본추출방법, 도구사용 승인여부, 리더십 도구의 신뢰도와 타당도, 연구설계 유형, 자료분석방법, 연구대상자, 대상자 수, 연구대상 기관, 대상기관 수, 자료수집 방법, 리더십의 변수 특성, 즉, 리더십이 연구에서 독립변수, 종속변수 등 각각 사용된 빈도, 세부 리더십 종류, 리더십 관련 주요 변수들과 리더십과 통계적 유의성을 가지는 변수로 구성되었으며, 분석도구의 각 항목별 세부 내용 및 작성근거는 선행연구[20, 26, 27, 28]를 참고하였다.
자료분석의 정확성을 높이고 오류를 줄이기 위해 연구팀이 여러 차례 함께 모여 분석 및 토론 등 공동 작업을 통해 자료를 추출하였고, 분석틀에 의한 자료분석 시 일관성을 유지하기 위해 연구자들이 독립적으로 논문을 분석하였으며, 분석 시 합의가 이루어지지 않는 등 논의가 필요한 사항에 대해서는 연구팀이 함께 토론한 후 의사결정을 하였다.
분석방법에 대하여 연구팀이 함께 토론하여 의사 결정한 세부적인 내용들은 다음과 같다. 도구의 신뢰도와 타당도의 경우, 분석대상 논문 본문에서는 신뢰도 또는 타당도를 확인했다고 기술되었으나, 실제 통계값이 제시되어 있지 않은 경우는 신뢰도 또는 타당도가 보고되지 않은 것으로 간주하였다. 연구대상 기관은 우리나라 의료법[29]에서 제시한 기준에 따라 분석 논문의 연구대상 기관을 중복 체크하였다. 대상논문에서 리더십과 관련되어 분석된 주요 변수를 조사할 경우, 예를 들어 셀프리더십과 관련하여 직무만족과 조직몰입을 관련변수로 설정한 경우, 한 편의 연구에서 각각 직무몰입 1건, 조직몰입 1건으로 처리하였으며 나머지도 이와 같이 분석하였다.
또한 분석된 논문의 상당수가 본문에서 제시된 한글도구 이름을 그대로 국문 제목에도 사용하였는데, 변수 중 한글도구 이름이 ‘간호업무성과’, ‘임상수행능력’, ‘간호업무수행’인 경우, 같은 도구를 사용한 연구일지라도 영문 제목은 차이가 있었다. 예를 들면 ‘간호업무성과’의 경우 영문제목은 각각 Nursing performance, Work performance, Nursing practice, Job performance, Clinical performance 등으로 제시되었으며, ‘임상수행능력’은 Nursing competence, Clinical performance 등으로, ‘간호업무수행’은 performance, Job performance, Nursing performance 등으로 영어 표기는 서로 혼재되어 사용하고 있었다. 이에 위의 세 가지 변수를 사용한 총 21건의 분석논문에 대해 각각의 연구가 도구 선정 시 참고한 문헌을 역으로 확인하여 실제 도구이름을 확인하였으며, 그 결과 도구이름을 ‘간호업무성과’ 라고 제시한 분석논문 13편 중 2편이 실제로는 ‘간호업무수행’ 도구를 사용하고 있었고, 1편은 ‘임상수행능력’ 도구를 사용하고 있었다. 이러한 결과를 토대로 기존의 한글 변수명을 실제 측정변수 중심으로 다시 코딩하였다.
또한 사용된 관련 변수의 도구가 거의 대부분 국내에서 만들어진 도구이기에 영문 도구이름은 따로 제시되어 있지 않았다. 이에 본 연구에서는 ‘간호업무성과’를 ‘Nursing performance outcome’으로, ‘임상수행능력’은 ‘Clinical practice competency’, ‘간호업무수행’은 ‘Nursing performance’로 통일하여 표기하였다. 매개변수로 사용된 경우에는 리더십이 각각 간호조직문화와 간호정보역량, 투약오류관리풍토와 오류보고 의도, 오류보고의도와 투약오류보고장애요인, 핵심자기평가와 혁신행동, 상사의 개인특성과 조직구성원의 조직시민행동, 직무 스트레스와 직무만족 사이의 매개효과를 검정하였으며, 조절변수로 사용된 경우에는 조직건강성과 고객지향성, 직무배태성과 간호업무성과, 간호업무성과와 자아상, 직무 스트레스와 조직몰입 사이의 조절효과를 검정하였다. 그 외 2편 이내의 연구에서만 사용된 관련 변수들을 기타로 분류하였다.
다음으로 분석한 논문 141편의 리더십 관련 특성은 리더십과 관련변수를 분석한 최종적인 통계분석에서의 p-value를 확인하였으며 통계적 유의수준은 p<.05로 설정하였다. 최종적인 통계분석방법이 상관분석인 경우 r값과 함께 제시된 p값을 확인하였으며, 회귀분석을 최종적인 통계분석방법으로 사용한 경우에는, 투입된 요인별 p값을 확인하였다. 구조방정식을 최종적인 통계분석방법으로 사용한 경우에는, 제시된 경로 중 직접효과 p값을 확인하였다. 유의성의 기준은 예를 들어 직무만족의 하위요인이 투입된 모형의 경우, 하위요인 중 일부가 유의하지 않으면 일부만 유의한 것으로 코딩하였으며, 하위요인이 모두 유의한 경우에만 전체 변수가 유의한 것으로 분석하였다.
이와 같이 세부적인 분석방법에 의하여 수집된 자료는 마이크로소프트 엑셀 프로그램 및 SPSS/WIN 23.0 프로그램을 통해 기술통계 및 다중반응분석을 이용하여 연구목적에 따라 빈도와 백분율을 산출하였다.
본 연구는 C대학교 생명윤리심의위원회의 승인을 받아 시행하였다(IRB No. 2018-04-016-001).
분석논문의 일반적인 특성은 Table 1과 같다. 전체 논문 141편의 연도별 분석편수는 1975~1990년 8편(5.7%), 1991~2000년 14편(9.9%), 2001~2010년 47편(33.3%), 2011~2018년 72편(51.1%)이었다. 전체 논문 141편의 제 1저자 직종은 교수인 경우가 97편(68.8%)으로 가장 많았고, 논문의 저자 수는 2명인 경우가 62편(44.1%)으로 가장 많았으며 평균 저자 수는 2.3명 이었다. 연구모형이 제시된 경우는 32편(22.6%)이었으며, 그중 31편은 기존 문헌고찰 내용을 바탕으로 연구자가 설정한 모형이었고, 1편만 Kerr와 Jemier의 리더십 대체이론에 근거하여 제시되었다. 연구가설이 제시된 경우는 27편(19.1%)이었고, 대다수인 114편(80.9%)은 가설이 설정되지 않았다.

표본 수 산정에서 산출기준을 제시한 경우는 55편(39.1%) 이었고, 효과크기를 제시한 경우는 42편(29.8%)이었다. 표본추출방법은 편의표집이 29편(20.6%)으로 가장 많았고, 표본추출방법이 보고되지 않은 경우가 103편(73.1%)이었다. 연구설계 유형을 살펴보면, 분석 논문 총 141편 중 136편이 양적연구였으며 그 중 예측연구가 85편(60.3%)으로 가장 많았고, 그 다음은 상관관계 연구 18편(12.8%), 경로 분석 연구 16편(11.3%)순이었다. 질적연구는 5편이었고 그 중 Q-방법론을 사용한 연구가 3편(2.1%)으로 가장 많았으며 포커스 그룹 인터뷰와 내용분석 연구가 각각 1편(0.7%)이었다.
리더십 도구사용 승인을 받은 경우는 13편(9.2%)이었으며, 연구자가 개발한 도구를 사용한 경우는 4편(2.8%)이었다. 시기별 변화를 보면 도구사용 승인을 받은 경우는 1975~2000년까지는 0편(0.0%), 2001~2010년 1편(0.7%), 2011~2018년 12편(8.5%)으로 점차 증가하였다. Q 방법론, 인터뷰 등 질적연구 방법인 경우와 리더십 도구 사용을 하지 않은 경우는 리더십 도구 사용을 하지 않았기에 도구사용 승인과 관련 없는 것으로 판단하였으며 이는 총 7편(4.9%)이었다. 리더십 도구의 신뢰도와 타당도에 대해 살펴보면, 양적연구를 기준으로 신뢰도가 보고된 경우는 124편(91.2%), 타당도가 보고된 경우는 19편(13.9%)이었다. 자료분석방법은 분석대상 논문에서 제시한 방법을 중복 체크하였으며 그 중 신뢰도 분석이 126편(89.3%)로 가장 많았고, 그 다음은 기술통계분석 125편(88.7%), 상관관계 분석 109편(77.3%) 순이었다.
연구대상자에 대해 분석한 결과 일반간호사 또는 간호관리자의 단일 직종을 대상으로 연구한 경우가 102편(72.5%)이었으며, 일반 간호사와 간호관리자 두 가지 직종을 모두 연구대상으로 분석한 경우는 39편(27.5%)이었다. 분석대상 논문에 포함된 연구대상자를 중복 체크하여 분석한 결과 일반 간호사를 대상으로 한 연구가 136편(96.5%), 간호관리자를 대상으로 한 연구가 48편(33.9%)이었고, 간호관리자를 다시 구체적으로 두직급으로 구분하여 분석한 결과 수간호사, 즉 일선관리자를 대상으로 한 연구가 42편(29.7%), 일선관리자보다 상급관리자를 대상으로 한 연구는 6편(4.2%)이었다. 대상자 수는 양적연구의 경우 평균 292.4명이었고 범위는 26명~1,216명으로 편차가 컸으며, 그 중 201~300명 사이가 52편(37.0%)으로 가장 많았다. 질적연구의 경우 평균 연구대상자 수는 26.8명이었다.
연구대상 기관은 우리나라 의료법[29]에서 제시한 기준에 따라 분석 논문의 연구대상 기관을 중복 체크하였으며, 그 결과 종합병원을 대상으로 한 연구가 65편(46.2%)으로 가장 많았고, 그 다음으로 상급종합병원 11편(7.7%), 병원 5편(3.5%) 순이었다. 의료법 기준에 따라 분류되지 않는 연구대상 기관으로는 대학병원이 49편(34.8%)으로 가장 많았고, 그 다음은 중소병원 4편(2.8%), 보훈병원 1편(0.7%) 순이었으며, 연구대상 기관이 제시되지 않은 경우는 21편(14.9%)이었다. 연구대상 기관 수는 평균 4.9개였으며 1~3곳인 경우가 71편(50.5%)으로 가장 많았다. 자료수집방법은 분석대상 논문에서 제시한 방법을 중복 체크하였으며 그 결과 설문지를 이용한 경우 139편(98.6%)으로 분석대상 논문 대부분이 자료수집방법으로 설문지를 이용하였고, 그 중 Q 방법론 연구 3편은 설문지와 인터뷰의 두 가지 방법을 사용하였다(Table 1).
전체 논문에서 연구된 리더십 종류는 Table 2와 같다. 논문에서 연구된 구체적인 리더십 종류는 각 연구마다 중복 체크하였으며, 그 결과 셀프리더십이 44편(31.2%)으로 가장 많았고, 다음은 변혁적 리더십 41편(29.0%), 다음으로는 일반적인 리더십 27편(19.1%) 등의 순이었다. 연도별로 보면 현재까지 44편이 발표된 셀프리더십 연구는 모두 2000년 이후로 이루어졌고, 변혁적 리더십은 1990년 이후로 연구가 발표되었으며 2001~2010년에 24편, 2011~2018년까지 13편 발표되었다. 그리고 국내 병원 간호사 대상 간호 리더십 연구 초기에는 특정 리더십으로 명명되지 않는 일반 리더십으로 연구가 수행되어 왔으며, 2010년 이후로는 일반 리더십으로 명명되는 경우가 3편으로 감소되었다. 또한 2010년 이후로는 윤리적 리더십 6편, 서번트 리더십 6편, 감성적 리더십 5편, 진성 리더십 4편 등 다양한 리더십의 연구가 발표되었다(Table 2).

전체 논문의 리더십 변수 관련 특성은 Table 3과 같다. 리더십의 변수의 역할을 연구에서 제시된 최종적인 통계분석방법에서의 리더십 역할에 따라 분석하였다. 최종적인 통계방법이 회귀분석, 경로분석 등 리더십의 역할을 명확히 원인과 결과로 구분할 수 있는 경우는 독립변수 또는 종속변수로 제시하였으며, 상관관계를 최종적인 통계방법으로 사용한 경우는 관계변수로 제시하였다. 예를 들어 셀프리더십인 경우 제목에서 표현된 리더십 변수의 역할뿐만 아니라 셀프리더십이 최종적인 통계분석방법에서 어떠한 역할을 하였는지를 파악하였다. 그 결과, 리더십 변수역할이 독립변수(independent variable)인 경우가 85편(60.3%)으로 가장 많았고, 관계변수(related variable)인 경우가 28편(19.9%), 매개변수(mediating factor)인 경우가 6편(4.3%), 조절변수(moderating factor)인 경우가 4편(2.8%)이었으며, 종속변수(dependent variable)로 사용된 경우는 5편(3.5%)이었다. 이 외에 해당되지 않는 변수역할인 경우 즉, 리더십 관련 요구도 조사, 유형연구, 중요도 분석, 도구개발 연구, Q 방법론, 인터뷰는 함께 묶어서 분류하였으며, 총 13편(9.2%)이 이에 해당되었다.

분석 대상 논문에서 리더십과 관계되어 분석된 주요 변수들을 확인한 결과는 Table 3과 같다. 논문에서 사용된 리더십 관련 주요 변수들에 대한 분석결과, 직무만족이 33편(25.8%)으로 가장 많이 연구된 관련변수로 나타났고, 그 다음은 조직몰입으로 28편(21.9%), 간호업무성과 10편(7.8%), 임파워먼트 10편(7.8%) 등의 순이었다. 그 외 2편 이하의 연구에서만 사용된 관련 변수들을 기타로 분류한 결과 총 69편(54.0%)이 해당되었다(Table 3).
리더십과 관련 변수들의 통계적 유의성을 분석한 결과는 Table 4와 같으며, 리더십과 다른 변수들의 관계가 3건 이상인 경우만 표에 제시하였다. Table 3에 제시된 결과를 셀프리더십을 예시로 설명하면 다음과 같다. 분석대상 141편의 논문에서 셀프 리더십을 연구 개념으로 한 경우는 총 44편이었으며, 44편을 100%로 보았을 때 직무만족은 그 중 25%(11편)에서 사용된 변수였다. 직무만족이 셀프리더십과 관련된 변수로 사용된 11편을 다시 100%로 보았을 때 그 중 7편(63.6%)의 연구에서 셀프리더십과 직무만족이 유의한 결과를 보였다.

또한 리더십 변수와 함께 분석된 일반적 특성 변수들 중 가장 많이 사용된 일반적 특성 변수는 나이, 학력, 임상 경력, 결혼상태, 근무부서, 직위 등이었다. 이들 일반적 특성과 리더십 변수와의 관계를 셀프 리더십을 예시로 설명하면 다음과 같다. 셀프 리더십 44편의 연구 중에서 일반적 특성 변수인 연령을 조사한 연구는 97.7%(43편)이었고, 조사한 43편을 100%로 보았을 때, 연령이 셀프 리더십과 p<.05 이상 유의한 결과를 보인 연구는 48.8%(21편)이었다. 또한 일반적 특성변수인 학력을 조사한 연구는 93.2%(41편)이었으며, 학력이 셀프리더십과 p<.05 이상 유의한 결과를 보인 연구는 39.0%(16편)이었다. 이와 같이 각 리더십별 가장 자주 사용된 일반적 특성변수 및 통계적 유의성을 순위별로 10위까지 Table 3에 제시하였다(Table 4).
본 연구는 국내 병원 간호사를 대상으로 이루어진 간호 리더십 연구의 동향을 파악하고자 시도되었으며, 본 연구에서 도출된 주요 결과를 토대로 다음과 같이 논의하고자 한다.
먼저 분석한 문헌들에서 연구모형을 제시한 연구는 전체 141편 중 총 32편(22.6%)이었으며 이는 국내 간호조직에서의 논문고찰 연구를 수행한 Kim 등[20]의 연구에서 이론적 기틀이 제시된 경우가 31.9%, 국내 학술지 게재논문을 분석한 Shin 등[27]의 연구에서 개념적 기틀을 제시된 경우가 23.7%[27]라고 보고한 결과와 유사하였다. 본 연구에서 연구모형을 제시한 32편 중 31편은 기존 문헌고찰을 바탕으로 연구자가 설정한 연구모형이었으며, 1편만 Kerr와 Jemier (1978) 리더십 대체이론에 근거한 연구모형이었는데, 이는 Shin 등[27]의 연구에서 42편 중 7편이 간호이론을 기반으로 개념적 기틀을 제시한 것과는 다소 차이가 있었다. 또한 1985년에서 2017년 사이 수행된 간호 분야의 리더십 연구를 체계적 문헌고찰을 통해 분석한 국외의 Cumming 등[11]의 연구는 전체 129편 중 113편의 연구가 이론적 기틀을 사용한 것으로 보고하고 있는데 이들 연구와 비교시 본 연구에서 분석한 국내 문헌들의 이론적 기틀 사용빈도는 매우 낮음을 알 수 있다. 그러므로 앞으로 국내 리더십 연구가 보다 발전하기 위해서는 연구하고자 하는 리더십에 대해 보다 더 명확하고 논리적인 연구를 위해 이론적 기틀[30]의 적용이 필요하다고 사료된다. 더불어 이론적 근거에 기반한 리더십 연구의 검증을 통해 보다 타당한 근거를 갖춘 간호 리더십 교육과 중재가 필요하다고 생각한다.
본 연구결과 양적연구에서 사용된 연구도구의 신뢰도를 보고한 경우가 124편으로 91.2%를 차지하였다. 이러한 결과는 Journal of Korean Academy of Nursing의 2007~2008년도 게재논문을 분석한 Shin 등[27]의 연구에서 제시된 80.1%와 유사하였으며, 국외 최근 30여 년간의 간호 분야 리더십 연구를 분석한 Cumming 등[11]의 연구에서 전체 129편 중 123편의 연구에서 리더십 신뢰도를 제시한 것으로 보고된 결과와도 유사하였다. 그러나 본 연구결과를 통해 제시된 신뢰도를 확인해 보았을 때, 명확하게 신뢰도를 제시하지 않은 경우가 많았다. 즉, 연구에서 사용한 도구의 신뢰도뿐만 아니라 선행연구 도구의 신뢰도 제시도 중요한 부분인데 그것이 생략되는 경우가 많았고, 특히 원 도구의 신뢰도는 제시된 경우가 극히 드물었다. 연구자들은 신뢰할 수 있는 측정값을 제공하는 도구를 필요로 하며[30], 신뢰도가 높은 도구들은 연구의 검정력을 증대시킬 수 있다[30]. 연구자들은 연구대상, 연구 셋팅, 연구 지역 등 연구자마다 다양한 특성을 지닌 연구 상황에 처하게 되는 데 각각의 상황에서 사용하는 도구가 얼마나 일관성을 가지는 지 확인하기 위하여 연구수행 시 사용하는 도구의 신뢰도 제시를 명확하게 할 필요가 있다.
연구설계방법으로는 대부분 양적연구방법이 많이 이용되었고(96.4%) 설문지를 활용한 자료수집방법이 139편(98.6%)을 차지했는데, 이는 Cumming 등[11]의 연구에서 자가보고의 방법을 사용한 연구가 전체 129편 중 110편(85.3%)으로 보고된 결과와 유사하였다. 그러나 병원 간호사의 리더십과 관련된 다양한 관점을 설문지에 제시된 문항으로만 파악하기에는 제한이 있을 수 있으며, 병원 간호사가 병원 등 조직에서 경험하는 여러 가지 맥락이나 상황, 그리고 간호관리자나 동료 간호사와의 관계에서 경험하는 여러 가지 리더십을 깊이 있고 심층적으로 이해하는 데는 제한이 있을 것으로 생각된다. 이는 Cumming 등[11]도 리더십이 간호의 성과에 미치는 실제적인 결과를 확인하기 위해서는 리더십 스타일에 대한 자가 보고 방식보다는 관찰 등을 활용한 연구설계가 필요하다고 제언한 바 있다. 따라서 추후 리더십 관련 연구에서는 설문지를 이용한 연구뿐만 아니라, 관찰이나 인터뷰 및 mixed-method research 등 다양한 연구방법을 활용하여 간호 리더십에 대해 분석하고 이와 연관된 성과를 보다 다각적인 관점에서 파악해 봄으로써 향후 간호 리더십 연구가 추구해야 할 방향을 모색해 볼 필요가 있겠다.
본 연구결과 연구가 진행된 기관으로 구분함에 있어서는 분류에 어려움이 있었는데, 그 원인은 각 문헌에서 의료기관의 표현이 다양하게 제시되었기 때문이다. 즉, 문헌들에서 표현된 기관의 특성이 현재 의료법에 제시된 의료기관의 종별 이외에 대학병원, 중소병원 등과 같이 표현되는 경우가 많았다. 우리나라 의료기관의 종별은 현재 의료법에 “의원, 치과의원, 한의원, 조산원, 병원, 치과병원, 한방병원, 요양병원, 종합병원, 상급종합병원, 전문병원”으로 구분되어 있다[29]. 그러므로 향후 연구들에서는 의료법에 명시된 우리나라의 의료기관의 종별을 정확하게 제시함으로써 연구의 신뢰성을 확보하기를 제언한다. 또한 본 연구에서 연구가 진행된 기관은 종합병원, 대학병원, 상급종합병원 순으로 많았는데 이는 그동안 리더십 연구가 주로 종합병원과 대학병원에 근무하는 간호사를 연구대상자로 선정하였음을 알 수 있다. 그러나 우리나라가 초고령화 사회로 향함에 따라 요양병원의 규모나 수가 점차 증가하는 추세이고, 이와 더불어 요양병원에 근무하는 병원 간호사수도 증가하고 있으므로[25] 추후 리더십 연구를 계획함에 있어 요양병원의 간호사를 대상으로 한 리더십 연구도 필요하다고 생각된다. 아울러 간호사에게 요구되는 리더십은 근무하는 병원 종별에 따라 차이가 있을 것으로 예상되므로 병원 종별로 중요하게 여겨지는 리더십 및 이에 따른 간호업무 성과를 비교분석하고 이에 적합한 중재방안을 모색해 볼 필요가 있다.
리더십 연구를 연도별로 살펴보면, 국내 병원 간호사들을 대상으로 하는 리더십 연구는 1970년대부터 연구되어 오기 시작하여 1990년까지는 8편(5.7%)에 불과하였으나, 1991~2000년 까지 14편(9.9%), 2001~2010년까지 47편(33.3%), 2011~2018년 현재까지 72편(51.1%) 등 2000년도 이후 진척을 보이고 2010년 이후부터 현재까지 연구가 활발하게 진행되고 있다. 이는 2006년까지 수행된 간호조직의 리더십 연구를 분석한 Kim 등[20]이 간호 분야 리더십 연구의 증가 추세를 5년 단위로 제시하였던 것과 같은 맥락이며, 2006년 이후로도 지속적으로 간호 리더십 연구가 수행되고 있었다는 것을 보여준다. 리더십 연구 초기였던 1970년대부터 리더십, 지도성 등으로 표현되어 진행되던 연구들은 2000년대를 거쳐 2010년대로 갈수록 하향화되는 추세를 보이는데 이는 전체적으로 리더십 연구가 감소된 것이 아니라 리더십 연구가 보다 더 세분화되고 정교화 되는 과정임을 보여주는 결과였다. 특히 리더십 종류별 연구 빈도를 살펴보면 전체 리더십 연구 중 셀프리더십, 변혁적 리더십 연구가 많이 이루어졌다. 그 중 셀프리더십이 상당한 빈도를 차지한 것은 급속한 병원환경의 변화 속에서 기존의 리더십으로는 조직의 목표를 달성하기 어려우며[4], 긴박하게 돌아가는 상황에 처한 간호사가 이에 대처하고자 스스로 리더십을 발휘하게 되는 최근의 경향이 반영된 결과로 보여진다[22]. 더불어 2010년 이후인 최근에는 윤리적 리더십 6편, 공감 리더십 5편, 서번트 리더십 6편, 진성 리더십 4편 등 다양한 유형의 리더십 연구가 등장하고 있다는 것은 간호 리더십에 대한 다양성이 추구되고 있다는 것이므로[4] 긍정적인 결과로 생각된다. 또한 리더십을 리더만이 갖추어야 할 역량으로 보기보다는 관리자가 아니더라도 각 간호사 개인이, 간호현장에서 환자 및 여러 관련부서 사람들과 업무를 진행하는 과정에서 갖추어야 할 역량으로 리더십에 대한 개념이 확장되어 온 것[12]과 맥을 같이 한다. 이와 더불어 최근 간호대학생의 셀프리더십에 대한 문헌을 분석한 Park 등[19]의 연구 및 간호사의 셀프리더십의 체계적인 고찰 및 메타분석 연구를 수행한 Yu 등[22]의 연구가 이루어진 것과도 유사한 흐름으로 생각된다.
연구대상자의 경우 복수 응답처리되었는데, 141편의 분석 논문 중에서 일반간호사가 포함된 연구가 96.5%, 간호관리자가 포함된 연구 33.9%였고, 관리자 중에서도 최고 관리자가 포함되는 경우는 극히 드물었고 일선관리자가 주로 관리자에 포함되었다. 각 연도별 리더십 연구 편수의 연구대상자를 확인한 결과, 최근 연구일수록 관리자가 리더십 연구에서 차지하는 비중이 줄어드는 것을 알 수 있다. 그리고 최근에 연구가 많이 되고 있는 셀프리더십은 자기개발의 측면이 강하므로[2, 22], 리더십 측정 및 프로그램 개발 등에 있어 주체자로서의 일반 간호사들에게 접근이 용이한 장점이 있을 것으로 생각된다. 그러므로 추후에는 이러한 일반 간호사들을 대상으로 하는 리더십 연구결과들을 확인하여 일반 간호사들을 위한 리더십 프로그램 개발이 필요하다고 사료된다.
분석 대상 논문에서 리더십과 관련되어 분석한 변수는 직무만족(25.8%), 조직몰입(21.9%), 간호업무성과(7.8%) 순으로 많이 연구되었는데, 이는 1975~2006년 사이 국내 간호조직의 리더십에 대해 학위논문과 학술지를 분석한 Kim 등[20]의 연구와 동일한 순서였다. 또한 이러한 결과는 국외에서 1985년에서 2017년 사이 수행된 간호 분야의 리더십 연구를 분석한 Cumming 등[11]의 연구에서 리더십과 관련된 변수로 직무만족과 조직몰입이 가장 많았던 결과와 일치한다. 비록 본 연구에서 분석된 논문들의 출판시기는 기존 연구들[11, 20]과 어느 정도의 시기적인 차이는 있지만 간호 리더십에 있어서 중요하다고 여겨지는 속성은 리더십 연구가 이루어진 1975년대부터 현재까지 크게 다르지 않음을 보여준다. 본 연구의 분석결과 직무만족, 조직몰입, 간호업무성과 등은 연구된 리더십 종류와 관계없이 거의 대부분의 연구에서 50% 이상 유의한 결과를 보이고 있었다. 이러한 결과는 다양한 특성을 가진 대상자, 다른 지역, 다른 셋팅에서 이루어진 연구임에도 불구하고 해당 변수들에 대해서는 제법 일관성 있는 결과를 예측할 수 있는 것으로 여겨진다. 이에 추후 연구자들은 이미 리더십과 유의한 관계가 있는 것으로 파악된 변수들을 반복 연구하기보다는 새로운 관점에서, 예를 들면 간호 리더십 개념분석 연구[13]에서 도출된 간호 리더십의 다섯 가지 속성인 “개인의 성장, 협력, 간호탁월성, 창의적 문제해결, 영향력” 등에 초점을 둔 연구를 계획하는 등 간호 분야만의 리더십을 보다 명료화하는 방향으로 연구를 계획할 필요가 있다고 사료된다. 반면, 적은 연구 편수에서 나온 결과에 대해서는 확대 해석하기 어려우므로 추후 반복연구가 필요할 수 있으며, 리더십과 관련변수들과의 관계가 서로 일치하지 않는 것으로 나타난 이직의도 등의 변수는 과학적인 분석 방법인 메타분석 등의 방법을 통해 확인해야 할 필요가 있다.
본 연구는 국내에서 수행되어 2018년 4월까지 국내 학술지에 발표된 병원 간호사 리더십 연구논문들을 분석하여, 국내 간호 리더십에 대한 동향을 확인하고 추후 리더십 연구의 기초자료를 제공하기 위한 연구이다. 본 연구의 제한점으로 본 연구는 국내의 검색엔진을 통해 추출된 문헌만을 대상으로 하였기 때문에 국외의 검색엔진을 통해 국외에서 출판된 국내 간호사 대상의 연구를 포함하지 못한 점, 학술지 논문 외에 학위논문을 포함시키지 못한 점이다. 그럼에도 불구하고 국내에서 수행한 간호 리더십 연구에 함유된 여러 가지 특성들을 파악함으로써, 현재까지 이루어진 리더십 연구의 전반적인 흐름을 확인하는데 기여하였다고 생각된다.
본 연구는 국내에서 병원 간호사를 대상으로 이루어진 간호리더십 동향을 파악한 서술적 조사연구이다. 본 연구결과, 간호 리더십 연구는 양적연구가 대부분이었으며, 분석대상 문헌에서 이론적 기틀을 사용하지 않는 경우가 많았다. 가장 많이 연구된 리더십 유형은 셀프리더십, 변혁적 리더십 순이었으며, 리더십과 관련된 주요 특성변수는 조직 관련 성과 중 직무만족, 조직몰입 등으로 파악되었으며, 일반적 특성변수로는 직위, 나이, 최종학력 등이었다. 이 외에도 국내 간호 리더십과 관련된 다양한 연구결과가 도출되었다. 본 연구는 국내에서 병원 간호사를 대상으로 수행된 리더십 연구의 장점 및 제한점 등을 전체적으로 조망하였으며 이러한 결과를 통하여 무의미한 반복연구를 지양하고, 리더십 관련 새로운 연구문제를 도출하기 위한 아이디어 자극 및 미래의 연구 방향을 정하는데 유용한 기초자료를 제시하였다는 점에서 연구의 의의를 갖는다.
이에 향후 연구를 위해 다음과 같이 제언하고자 한다. 첫째, 이론적 기틀에 기반을 둔 간호 리더십 연구수행을 제언한다. 둘째, 간호 리더십의 다양한 속성을 확인하기 위해 실험연구, 질적연구, 메타분석, 구조방정식 등 다양한 방법론을 활용한 연구를 제언한다. 셋째, 요양병원 셋팅에서의 간호 리더십 연구를 제언한다. 넷째, 일반 병원 간호사를 위한 리더십 개발 프로그램을 개발하고 그 효과를 확인하는 연구를 제언한다.
