Purpose
This study investigated the mediating effects of patient safety management activities on the relationship between nurse-nurse collaboration, nurse-physician collaboration, and nursing performance of clinical nurses. Methods Online survey was performed from February 18 to February 28, 2023 using structured questionnaires. The participants were 212 clinical nurses working in tertiary general hospitals in South Korea. The participants completed self-reporting questionnaires, that measured nurse-nurse collaboration, nurse-physician collaboration, nursing performance, and patient safety management activities. Data were analyzed using SPSS 29.0 program, for multiple regression and a simple mediation model, applying the PROCESS macro with a 95% bias-corrected bootstrap confidence interval. Results Nurses' patient safety management activities had a mediating effect on the relationship between nurse-nurse collaboration and nursing performance (B=0.24, Boot 95% CI=0.16∼0.34). In addition, patient safety management activities showed a mediating effect on the relationship between nurse-physician collaboration and nursing performance (B=0.10, Boot 95% CI=0.07∼0.15). Conclusion The levels of nurse-nurse collaboration, nurse-physician collaboration, and patient safety management activities must be considered when developing strategies to improve nurses’ performance in nursing practice settings. Purpose
This study aimed to identify the effects of resilience and nursing managers’ empowering leadership on turnover intention among new nurses, as well as the mediating role of transition shock. Methods This was a descriptive, cross-sectional study. Data were collected from 167 new nurses working in tertiary hospitals in Korea between July 1st and July 13, 2022. The relationships among the variables were analyzed using Pearson’s correlation coefficients, and the factors affecting turnover intention were identified using PROCESS macro analysis. Results Resilience (direct effect=-0.02, p=.781) and nursing managers’ empowering leadership (direct effect=-0.00, p=.992) did not directly affect new nurses’ turnover intention. Transition shock had mediating role in the relationship between resilience and turnover intention (indirect effect=-0.12, 95% Boot confidence interval=-0.23~-0.04) and between nursing managers’ empowering leadership and turnover intention (indirect effect=-0.14, 95% Boot confidence interval=-0.23~-0.06). Conclusion Resilience and nursing managers’ empowering leadership indirectly influence turnover intention through transition shock among new nurses. To reduce turnover intention among new nurses, programs for preventing or mitigating their transition shock should be provided. In addition, improving the resilience of new nurses and nursing managers’ empowering leadership may contribute to reducing turnover intention among new nurses by mitigating their transition shock.
Purpose
This study aimed to determine the mediating effect of leader-member exchange (LMX) on the relationship between nursing unit managers’ethical leadership and nurses’turnover intentions. Methods A cross-sectional nationwide survey was performed using structured questionnaires. A total of 225 clinical nurses working in general tertiary hospitals were recruited through proportional quota sampling; the quota used represented the location of working hospitals in South Korea. Data were collected through an online survey and snowball sampling from June to September 2022 and analyzed using the SPSS 25.0 program. A mediation analysis was performed using a simple mediation model applying the PROCESS macro with 95% bias-corrected bootstrap confidence interval. Results In bootstrapping, the indirect effect (X→M→Y) was statistically significant (B=-0.77, 95% CI:-1.02 to -0.50). LMX showed a mediating effect on the relationship between nursing unit managers’ethical leadership and clinical nurses’ turnover intention. In this model, the total effect was B=-0.93 (p<.001). Conclusion These findings suggest that LMX must be considered when making a strategy for nursing retention in the hospital setting. Nursing unit managers with ethical leadership could decrease nurses’ turnover intention by improving the LMX.
Purpose
This study investigated the effects of job embeddedness and resilience on the relationship between job rotation stress and nursing work performance among clinical nurses. Methods: We conducted a nationwide online survey with 205 nurses employed at advanced general or general hospitals. Data were collected from August 17 to October 5, 2021. We analyzed the data using various descriptive statistics, independent t-tests, one-way analysis of variance, Scheffé test, Welch test, Games-Howell test, Pearson's correlation coefficients, multiple regression, and the PROCESS macro with a 95% bias-corrected bootstrap confidence interval. These analyses were conducted using SPSS Windows Software version 23.0 and SPSS PROCESS macro version 3.5.2. Results: Nurses' job embeddedness mediated the relationship between job rotation stress and nursing work performance (B=-0.02, p<.05). Furthermore, their resilience moderated this mediated relationship, also known as moderated mediation, with job embeddedness as the mediating factor (B=-0.02, p<.05). Conclusion: These findings imply that, when devising a strategy to improve the nursing work performance of nurses undergoing job rotation, their resilience levels should be considered. Reducing job rotation stress while concurrently enhancing job embeddedness is essential for enhancing the nursing work performance of nurses with high levels of resilience.
Purpose
This study examined the effect of missed nursing care on nursing sensitive indicators. Methods A nationwide cross-sectional survey was conducted using structured questionnaires. Data of 174 clinical nurses employed in general or tertiary hospitals were collected through proportional quota sampling; the quota used was the location of a working hospital in South Korea. Data were collected through an online survey and snowball sampling from July 16 to July 20, 2022. The collected data were analyzed using IBM SPSS 26.0. Results Missed nursing care had statistically significant negative correlations with patient safety management activity and job satisfaction, and significant positive correlation with turnover intention. Significant correlation was not found between missed nursing care and adverse event experiences. Hierarchical multiple regression analysis revealed that missed nursing care explained an additional 21%p of patient safety management activity, 14%p of job satisfaction, and 3%p of turnover intention. Thus, missed nursing care was found to be a statistically significant predictor of patient safety management activity, job satisfaction, and turnover intention. Conclusion Missed nursing care significantly affects nursing sensitive indicators. To improve positive outcomes and decrease negative outcomes, nurses and nursing managers must make efforts to minimize missed nursing care. Citations Citations to this article as recorded by
Purpose
The aim of this study was to explore the causes and effects of burnout experienced by insurance review nurses working in hospitals. Methods: Data were collected from April to May, 2021, using focus group interviews. The study enrolled 19 insurance review nurses with at least one year of experience. Data were analyzed using the content analysis method. Results: Causes of burnout were divided into three categories: eight subcategories (feeling of insufficient competence in an insurance review, closed mind in a limited working space, incompetency from other departments, harsh words and deeds that exclude me, insurance cuts controlling my work performance, cloudy mind chased by the billing date, ineffective hospital information systems, and question with no answer) and 23 causes. The impact of burnout was divided into three categories: six subcategories (experiencing mental and physical symptoms, projecting one’s feelings into family or neglecting them, withdrawing myself, wearing a mask, decreased work performance, and desiring to leave one’s work), and 14 concepts. Conclusion: This study provides basic data for strategy development to prevent and solve burnout problems among insurance review nurses. Purpose
This study was done to investigate the mediating effect of trust in supervisor in the relationship between ethical leadership of nursing unit managers and job embeddedness of staff nurses. Methods: Participants were 216 staff nurses from one tertiary general hospital and three general hospitals. Data were collected from June to August, 2018, using self-report questionnaires. Data were analyzed using descriptive statistics, independent t-test, Mann-whitney U test, One-way ANOVA, Kruskal-wallis, Welch test, Scheffé test, Games-Howell, Pearson correlation coefficients and multiple regression, with the SPSS/WIN 23.0 program. A mediation analysis was performed according to the Baron and Kenny method and Sobel test. Results: There were significant relationships between ethical leadership and trust in supervisor (r=.85, p<.001), ethical leadership and job embeddedness (r=.38, p<.001), and trust in supervisor and job embeddedness (r=.41, p<.001). Trust in supervisor showed full-mediating effects in the relationship between ethical leadership and job embeddedness. Conclusion: The results of this research show the importance of the role of trust in the supervisor as a full mediating variable. To increase the nurses’ job embeddedness, it is necessary to develop programs and strategies to enhance trust in supervisors, as well as training nursing unit managers to provide ethical leadership. PURPOSE
This study was done to identify trends in leadership-related research by reviewing studies on hospital nurses in South Korea. METHODS The research was conducted from April 2 to 14, 2018 and search databases were RISS, KISS, DBpia, KM base, NAL, and NDSL. Search terms were ‘leadership’ and ‘nursing’ in the Korean language, and total 141 papers were selected. RESULTS Analysis of the research on leadership showed that 96.4% of research methods were quantitative research, and 60.3% were predictive researches. For study settings, 46.2% were general hospitals, 96.5% were staff nurses. In the study of leadership variables, 60.3% were independent variables and recent leadership was used as a mediating variable (4.3%) and a moderating variable (2.8%). Among the leadership types, self-leadership (31.2%) was the most common, and the main variable related to leadership was job satisfaction (25.8%). CONCLUSION These results provide useful data for deriving new research ideas about nursing leadership. PURPOSE
This study was done to identify the effects of nurse-physician collaboration on nursing performance and organizational commitment in intensive care unit (ICU) nurses. METHODS A cross-sectional survey was used and data were collected from 203 ICU nurses working in two advanced general hospitals and two general hospitals. Data were analyzed using descriptive statistics, independent t-test, One-way ANOVA, Scheffé test, Pearson correlation coefficients and Hierarchical multiple regression with SPSSWIN 23.0 program. RESULTS The sub-domains of nursephysician collaboration were as follows: 3.77±0.51 for sharing of patient information, 3.36±0.65 for decision-making process on the cure or care, and 3.20±0.72 for relationship between nurse and physician. Nurse-physician collaboration was significantly positively correlated with nursing performance and organizational commitment. Regression analysis showed that nurse-physician collaboration explained an additional 24.9%p of nursing performance and an additional 13.4%p of organizational commitment. 'Sharing of patient information' and 'Relationship between nurse and physician' were significant predictors of nursing performance. 'Relationship between nurse and physician' was a significant predictor of organizational commitment. CONCLUSION These results provide evidence that the collaboration between nurses and physicians should be enhanced in terms of sharing patient information and mutual respect, to improve nursing performance and organizational commitment of ICU nurses. PURPOSE
The aim of this study was to identify effects of perceived nurse-nurse collaboration and nurse-physician collaboration on nursing performance in perioperative nurses. METHODS A cross-sectional survey was used and data were collected in September 2016. Participants were 186 perioperative nurses from three advanced general hospitals and nine general hospitals. Data were analyzed using descriptive statistics, independent t-test, One-way ANOVA, Scheffé test, Pearson correlation coefficients and hierarchical multiple regression with the SPSS/WIN 23.0 program. RESULTS The mean scores were for nurse-nurse collaboration, 2.92±0.28 out of 4, for nurse-physician collaboration, 3.29±0.65 out of 5, and for nursing performance, 3.85±0.47 out of 5. There were statistically significant positive correlations among nurse-nurse collaboration, nurse-physician collaboration, and nursing performance. Hierarchical multiple regression analysis revealed that nurse-nurse collaboration explained an additional 29%p of nursing performance. Shared processes, conflict management, and professionalism of nurse-nurse collaboration were statistically significant predictors of nursing performance. The nurse-physician collaboration explained an additional 3%p of nursing performance. Sharing of patient information was a statistically significant predictor of nursing performance. CONCLUSION Findings indicate that intervention programs that integrate and strengthen shared processes, conflict management, professionalism, and sharing of patient information are useful to enhance nursing performance. PURPOSE
The purpose of this study was to identify the importance, performance and rates of nurses' performance of nursing interventions in long-term care hospitals. METHODS A cross-sectional descriptive study was used and data were collected in 2015. Participants were 174 clinical nurses working in 14 long-term care hospitals. The collected data were analyzed using the SPSS/WIN 18.0. RESULTS The nurses' perceived importance of nursing interventions was 4.48±0.51, and the degree of nursing intervention performance in long-term care hospitals was 3.97±1.07 out of 5. The rates of nursing interventions performed by nurses ranged from 33.9% to 99.4% with an average of 82.5%. There were statistically significant positive correlations among perceived importance, performance in hospitals, and rates for nurses' performance of nursing interventions (p<.001). CONCLUSION The results indicate that interventions with a high degree of importance and performance could be the core nursing interventions in long-term care hospitals. These core nursing interventions should be included in education for nurses. These findings can be used in developing realistic guidelines and effective strategies for nurses and administrators to improve the quality of nursing and the status of nursing professionals in long-term care hospitals. PURPOSE
This study aimed to identify research trends in the Journal of Korean Academy of Nursing Administration from 2013 to 2015. METHODS For this study, 171 articles were analyzed. Research designs, participants, research settings, sampling, and data analyses methods were reviewed using established analysis criteria. Keyword centrality and clusters were generated by keyword network analysis. RESULTS Most of studies used quantitative methods (82.5%), and sampling mainly focused on nurses (68.8%). The most commonly used data analyses methods were t-test, ANOVA, correlation, and regression. The most central keywords were turnover and empowerment. Network analysis generated four network groups: 1) burnout; 2) turnover; 3) happiness; and 4) nursing professionalism. CONCLUSION The results of this study identify current trends and interests in Korean nursing administration research. PURPOSE
This study was conducted to develop a nursing leadership program outcome evaluation system, required for accreditation of nursing education. METHODS A methodological design was used. To ensure the theoretical validity of the evaluation system, learning objectives for nursing education programs and job descriptions for nurses in Korea were verified by analyzing the relationships in the five attributes of the nursing leadership concept. The nursing leadership program outcome evaluation system was developed based on the Kim & Park's developmental model (2008). RESULTS The nursing leadership program outcome evaluation system was established, including implementation level, education curriculum, level of performance, evaluation method, rubrics, and Close-the-Loop. CONCLUSION The developed evaluation system can be actively used in nursing education, and contribute to enhancing the leadership competencies of nursing students and graduate nurses. PURPOSE
The purpose of this study was to provide a concept analysis of nursing leadership. METHODS The concept analysis process of Walker and Avant (2011) was used. A review of the literature was conducted using several databases, including PubMed, CINAHL, Ovid-Medline, DBpia, and KISS. The databases were searched using the following terms 'nursing leadership', 'nursing' & 'leadership', and 'nurse & 'leadership', and for studies published from January 2000 to March 2015. Strict inclusion and exclusion criteria were applied. RESULTS Analysis of these studies indicated nursing leadership attributes were individual growth, collaboration, nursing excellence, creative problem solving, and influence. The antecedents of nursing leadership were the individual maturity level, construction of mutual trust and confidential relationships, and a job requirement. The consequences were improvement in nursing productivity, development of a supportive work environment, and satisfaction with the nursing profession. CONCLUSION The findings in this study provide a definite concept of nursing leadership. The findings also provide the theoretical evidence for the development of nursing leadership competencies in nursing students and professional nurses. PURPOSE
The purpose of this study was to analysis the manuscripts rejected for publication in the Journal of Korean Academy of Nursing Administration during the last three years six months (2012~2015 Jun). METHODS Seventy eight rejected manuscripts were analyzed focusing on manuscripts characteristics, such as rejection rates, year of submission, occupation of first author and corresponding author, number of authors, funding, thesis or dissertation, article type, study participants, number of reviews prior to rejection, and 3rd reviewer. Also reviewers' quantitative evaluation scores and subjective comments were analyzed. Reviewers' subjective comments were analyzed using content analysis methodology. RESULTS The mean rate for manuscript rejection was 28.9% and for quantitative research, qualitative research, and review papers the quantitative evaluation scores were 2.54+/-0.70, 2.39+/-0.69, and 2.39+/-0.69 out of 5 points, respectively. The most frequent subjective comment on rejected manuscripts was 'lack of rationale for research need'. CONCLUSION In this study the characteristics and the reasons for rejecting manuscripts were identified. These findings can be used in developing effective strategies for researchers, reviewers and editors to improve the quality of research and research reviews of nursing administration research. PURPOSE
This study was done to identify the experiences of ethical issues and needs for ethics education in clinical nurses. METHODS A cross-sectional survey was used and data were collected in 2013. Participants were 428 clinical nurses working in the general units of seven medical hospitals. The Ethical Issues in Clinical Practice Tool was used. Data analysis was performed using SPSS/WIN 19.0. RESULTS 'Providing care with a possible risk to your health' was the most frequent and disturbing ethical problems for nurses. The highest helpful ethical topic was 'the patients' right, autonomy and informed consent'. The ethical issue experience was significantly different according to education level, work units, and type of employment. The necessity of ethics education was statistically different according to age, religion, level of education, duration of working as RN, position, shift type, and continuing education about nursing ethics. CONCLUSION The results of this study show that nursing educators need to provide practical ethics education based on frequent ethical issues and helpful education topics. These findings can be used in developing effective education strategies for clinical nurses and nursing organizations to improve nurses' ethical decision-making abilities. PURPOSE
This study aimed to investigate job satisfaction, organizational commitment and turnover intention among male nurses in South Korea. METHODS Data were collected in May, 2013 and a total of 150 male hospital nurses participated in this study. Job satisfaction, organizational commitment and turnover intention were measured using a structured questionnaire. Collected data were analyzed using SPSS/WIN 20.0 for windows. RESULTS There were statistically significant correlations between job satisfaction and organizational commitment (r=.74, p<.001), job satisfaction and turnover intention (r=-.56, p<.001), and organizational commitment and turnover intention (r=-.69, p< .001). There was a statistically significant difference in job satisfaction of participants according to education, work unit, and salary. Organizational commitment showed significant difference according to age, education, work unit, and salary. Turnover intention was significantly different according to duration of employment in the hospital, and salary. The most influential factor for turnover intention was organizational commitment (beta=-.69, p<.001). This factor accounted for 47.8% of the variance in turnover intention. CONCLUSION The results of this study indicate that to reduce turnover intention for men in nursing, it is necessary to increase job satisfaction, and organizational commitment. PURPOSE
This study was conducted to test the effect of Image Making Programs on image making efficacy, positive thinking, self-esteem, and nursing professionalism in nursing students. METHODS A non-equivalent control group pretest-posttest design was used. Participants were 124 nursing students at two universities, and were assigned to the treatment group (n=62) or the comparison group (n=62). The treatment was the Image Making Program, which was held twice over 2 days for 120 minutes per session. Data were collected from August to September 2012, and were analyzed using descriptive statistics, Kolmogorov-Smironov test, chi2-test, independent t-test, Mann-Whitney U test, one-tailed Mann-Whitney U test, independent one-tailed t-test with the SPSS/WIN 21.0 program. RESULTS Nursing students in the treatment group showed statistically significantly higher levels of image making efficacy, positive thinking, and nursing professionalism than those in the comparison group. CONCLUSION The results indicate that the Image Making Program is an effective intervention for increasing image making efficacy, positive thinking, and nursing professionalism in nursing students. PURPOSE
The purpose of this study was to identify the major trends in research studies in the Journal of Korean Academy of Nursing Administration from 2010 to 2012. METHODS A review using analysis criteria developed by researchers was done of 132 studies published between 2010 and 2012. Research design, participants, research domain, and keywords were analyzed from the Journal of Korean Nursing Administration. RESULTS Job satisfaction, stress, organizational commitment, safety, turnover, nursing education, and performance were found to be major keywords. Of the research in the Journal, quantitative methods were used in 93.2% of studies. The major setting and participants were hospitals (58.2%) and nurses (65.5%) respectively. Prevalent analysis methods used were t-test, ANOVA, correlation, regression, chi-square, AMOS, and factor analysis. Major domains in the articles were: controlling, directing, staffing, nursing management education, and professionalism & legal principles. CONCLUSION Through this study, the research trends in nursing administration were identified, but there is a need to include more of the following topics in future research: new concepts in nursing policy, enhanced deliberations of IRB, rationalization of the effects in sample size calculations, theoretical development of planning and organizing, and development of interventions for management support of the nursing management process. PURPOSE
This study was done to develop a realistic clinical case and investigate nurses' decision-making about nurses' ethical dilemmas with physicians in the fields of nursing practice. METHODS Case development and a hypothetical case study were used. Participants were 52 nurses. Data were collected in 2012 and 2013 using an open-ended questionnaire and interviews and analyzed using content analysis and descriptive statistics. RESULTS Various dilemma situations between nurses and physicians, such as violence, deathbed, medication-prescription, and physicians' incapacity-unfairness, were suggested. A clinical dilemma case about medication-prescription was developed based on nurses' experiences. Nurses' responses to the developed case situation and responses were classified into five types. Various reasons were given for making the decisions and about 56% of the nurses decided to notify their supervisor without deleting nursing records. CONCLUSION In this study, a realistic clinical dilemma case was developed, and nurses' ethical decision making was identified. PURPOSE
This study was designed to describe the burnout experiences of nurses counseling people living with HIV/AIDS (PLWHA) in hospital-based counseling services in Korea. METHODS A qualitative research technique with focus group interviews was used and the data were collected in 2012. Participants were 13 counseling nurses from 16 medical hospitals in South Korea who had worked full-time for more than six months as a counseling nurse for PLWHA. The 13 nurses made up 68.4% of all 19 official HIV counseling nurses in South Korea. Data were collected using focus group interviews and analyzed with the content analysis methodology of Downe-Wamboldt. RESULTS Burnout experience of counseling nurses for PLWHA was analyzed for two domains-causes of, and effects of burnout. Each domain was classified into three categories; personal, job-related, and relationships. Further 17 sub-categories and 47 concepts were discovered. CONCLUSION In this study various causes and effects of burnout experienced by nurses counseling PLWHA were identified. These findings can be used in developing effective strategies for nurses, administrators, and health policy makers to solve the burnout of counseling nurses. PURPOSE
This study was done to provide information on issues of nursing core competencies needed in the fields of nursing practice for graduates in nursing, from the perspective of nursing managers, and to make suggestion for improving nursing education programs. METHODS Ten nursing managers participated in this study. They were in charge of clinical nursing education in the fields of nursing practice. Data were collected using focus group interviews and analyzed with the content analysis methodology of Downe - Wamboldt's. RESULTS Six types of nursing core competency - therapeutic nursing competency, professional nursing competency, administrative nursing competency, humanistic nursing competency, relational nursing competency, and personal nursing competency - were identified as nursing core competencies needed in the fields of nursing practice for graduates in nursing. CONCLUSION Results of the study show important evidence for decision-making about nursing curriculum revision based on nursing core competency, both in the classroom and in nursing practice areas. These results should contribute to the development of evaluation indicators for nursing students or new nurses. Further research is required to measure degree of nursing core competency in graduates of nursing and to identify the effect of competency-based education for improving nursing core competency. PURPOSE
This study was conducted to develop an evaluation framework for QI activity in medical institutions and to analyze QI activity cases by applying the developed evaluation framework. METHOD A four-phase process was employed to develop the evaluation framework, and a descriptive survey was used for the QI case study. Data were collected in April, 2010 by examining 157 QI activity cases presented at conferences and published in Journal of Korean Society of Quality Assurance in Health Care over the past three years. Developed QI activity evaluation instruments were used for data collection. Data were analyzed using the SPSS 18.0 for Windows program. RESULT A QI Activity Evaluation Framework was developed. This framework consisted of 45 items. The department with the highest level of QI participation was the nursing department. The most frequent QI activity theme was patient safety. QI activity levels in Korean medical institutions are relatively equalized without significant differences according to institution characteristics. CONCLUSIONS From the quality aspect of QI activity, more systematic and scientific approaches are required to upgrade QI activity. This study could provide methodological guidelines for QI activity and be useful in setting goals and directions for QI activity in medical institutions in Korea. PURPOSE
This study was done to examine effects of task performance style, communication ability and their interaction on problem-solving ability and nursing competency of nursing students participating in a nursing management practicum. METHODS The study was a non-equivalent control group non-synchronized design. Participants were 56 fourth year nursing students (25 in the cooperative task group and 31 in the individual task group) and data were collected from March to September 2010. Additionally, two groups were classified based on communication ability of students and four groups were classified by their task performance style and communication ability. Problem-solving ability and nursing competency were measured pre- and post-test and compared between groups. Data were analyzed using SPSS Windows 17.0 program. RESULTS Neither problem-solving ability and nursing competency were statistically significantly different according to task performance style. Nursing competency was statistically significantly higher in the high communication group compared to the low communication group. Problem-solving ability was significantly different among the four groups classified by task performance style and communication ability. CONCLUSION Nursing educators may need to improve students' communication ability to improve nursing competency and also assign different tasks based on communication ability of nursing students to improve problem-solving ability.
