Soon Min, Hye Sook Kim
J Korean Acad Nurs Adm 2012;18(4):374-382.   Published online December 31, 2012
DOI: https://doi.org/10.11111/jkana.2012.18.4.374
PURPOSE
This study was done to investigate the effects of university hospital nurses' organizational conflict between organizational commitment and labor union commitment, so as to provide data on prevention of conflict with the hospital and to improve work achievement with commitment of two different groups: hospital and labor union commitment.
METHODS
Data collection was conducted from May 15 to 31, 2011 for nurses registered in the labor union of a university hospital. The collected data were analyzed using t-test, ANOVA, Scheffe test and multiple regression.
RESULTS
Organizational commitment of the nurses showed significant differences according to age, position and work experience, and degree of labor union commitment according to academic achievement. The factors affecting labor union commitment were organizational conflict and recognition of need for labor union, which accounted for 19% of the variance. Factors affecting organizational commitment were organizational conflict, recognition of need for labor union, participation in organizational events, and renewal of membership in the union, which accounted for 33% of the variance.
CONCLUSION
The results of this study indicate that there is a need to improve work achievement by minimizing conflict and preventing labor disputes for better organizational commitment and labor union commitment of nurses.

Occupational Stress and Coping Styles as Factors Affecting the Burnout of Clinical Nurses
Kyoung Ja Kwon, Seung Hee Lee
J Korean Acad Nurs Adm 2012;18(4):383-393.   Published online December 31, 2012
DOI: https://doi.org/10.11111/jkana.2012.18.4.383
PURPOSE
The aim of this study was to investigate the effects of occupational stress and coping styles on burnout of clinical nurses.
METHODS
The participants in this study were 397 nurses, working at the hospitals in Seoul, Gangwon, and Gyongbuk provinces. Data were collected using a structured questionnaire from November, 2009 to January, 2010. The SPSS WIN 13.0 version program was used for data analysis.
RESULTS
The most significant predictors of burnout were positive reappraisal and work load. Work load, resource inadequacy, role ambiguity, and indifference coping style positively correlated with burnout. Positive reappraisal and problem-focused coping styles negatively correlated with burnout.
CONCLUSION
The study results indicate that it is important to reduce work load, resource inadequacy, and role ambiguity in nurses and to strengthen their positive reappraisal and problem-focused coping styles to prevent burnout. This could be achieved with job redesign, modification of shift work systems, and by offering burnout prevention program.

Determinants of Amount of Service Use in Community-Based Long-term Care for Elders
Taewha Lee, Bok Nam Kim
J Korean Acad Nurs Adm 2012;18(4):402-413.   Published online December 31, 2012
DOI: https://doi.org/10.11111/jkana.2012.18.4.402
PURPOSE
This study was done to explore factors related to amount of service use for elders with long-term care needs.
METHODS
A descriptive-correlation design was used. The sample included 259 elders and their primary caregivers who had cared for the elders for at least 6 months. Data on long-term care need assessment, service use and interviews with primary caregivers were analyzed.
RESULTS
There was no significant relationship between the sociodemographic characteristics and the amount of services use. Amount of service use differed significantly by Long-term care classification. The mean scores for class 1, 2 and 3 were 22.68, 21.47 and 17.87 days respectively. Primary caregiver relationship with the elders and the number of family-friend helpers were also significant. Multivariate regression analysis showed that gender, marital status, activities of daily living, cognitive impairment, and secondary caregiver support explained 17% of the total variance of service use among these elders (F=3.50, p<.001).
CONCLUSION
The results of this study indicate that critical factors including secondary caregiver support and individual background, and other functional dependencies except for physical function should be considered in accurately predicting the amount of service use for community dwelling elders with long-term care needs.

Impact of Unit-level Nurse Practice Environment on Nurse Turnover Intention in the Small and Medium Sized Hospitals
Jeong Ok Kwon, Eun Young Kim
J Korean Acad Nurs Adm 2012;18(4):414-423.   Published online December 31, 2012
DOI: https://doi.org/10.11111/jkana.2012.18.4.414
PURPOSE
The purpose of this study was to investigate the impact of the unit-level nurse practice environment on nurse turnover intention in the small and medium sized hospitals.
METHODS
A cross-sectional study was conducted using a questionnaire survey with 308 nurses in 38 nursing units of 6 small and medium sized hospitals, having over 200 beds and under 300 beds and located in B metropolitan city. Data were collected from July 20 to August 10, 2011. Data were analyzed using hierarchical multiple regression.
RESULTS
The mean turnover intention in nurses of small and medium sized hospitals was 3.52+/-0.53. Factors affecting turnover intention in the nurses included age, work unit, monthly income, number of night-duties, work hours per day and unit-level nurse practice environment. The unit-level nurse practice environment accounted for 15% of turnover intention when other variables were controlled.
CONCLUSION
The results of the study indicate that nurse turnover intention is associated with the nurse practice environment at the unit level. Small and medium sized hospitals can improve nurse retention and lower turnover intention by changing the nurse practice environment of unit, such as creating better support services and nurse participation in hospital affairs.

Mi Yu, Miok Kim
J Korean Acad Nurs Adm 2012;18(4):424-433.   Published online December 31, 2012
DOI: https://doi.org/10.11111/jkana.2012.18.4.424
PURPOSE
With the rapid increase in information technology in the medical environment, protection of patient's privacy is a crucial issue to hospital nurses. The purpose of this study was to determine neonatal nurses' perception and performance of behavior to protect patient privacy, and professional self-concept, and to investigate the relationships among these variables.
METHODS
The participants in this descriptive correlation study were 182 nurses in nursery and neonatal intensive care units using EMR or OCS in November, 2011.
RESULTS
Perception and performance averaged 4.61 and 4.04 (out of 5) respectively, and the average score for professional self-concept was 2.73 (out of 4). There was a significant difference between perception and performance. The performance of behaviour to protect patient privacy had a positive correlation with perception and professional self-concept. Multiple regression analysis showed that the key determinants of performance were recognition of necessity of patient privacy education, professional self-concept and perception, and these explained 36% of the total variance of performance.
CONCLUSION
Study results indicate a need to establish policy to protect privacy of neonates and their families, and to develop educational programs to enhance neonatal nurses' perception and performance.
