Informed Consent: A Complex Process in Iran's Nursing Practice

Informed consent should not be regarded as a rigid process. Instead, it should be regarded as a flexible process that provides patient with the right amount of information required to facilitate meaningful decision-making. Informed consent is an important ethical and practical part of patient care. Understanding the moral foundation of informed consent can help to guide therapists in their communication with all patients. Nurses, as members of the multidisciplinary team, are currently key contributors in providing information to patients regarding the treatment process. But in this process, there are some challenges that make it difficult. This paper describes the underlying ethical and regulatory requirements as well as related challenges regarding informed consent in the context of a nursing practice in Iran.

PURPOSE
The purpose of this study was to provide basic research material necessary for the establishment of comprehensive nursing service units, through a comparative analysis of inpatient satisfaction with nursing between comprehensive nursing service and general units and the work stress of nurses.
METHODS
The survey participants were inpatients and nurses from 3 general hospitals in Busan, including 6 units, 123 nurses and 220 patients. Date collection was done through March and April 2016. Collected data were analyzed using test and independent t-test with SPSS 23 Win program.
RESULTS
Inpatients on comprehensive nursing service units had higher scores for satisfaction than inpatients on general units. There was no statistically significant difference in nurses' work stress between the two types of units.
CONCLUSION
The findings indicate that although inpatient satisfaction with comprehensive nursing service units is higher than for general units, work stress for nurses is comparatively high for both types of units. While it is necessary to expand the implementation of comprehensive nursing service units, an institutional strategy for reducing nurses' work stress is required if the expansion is to be successful.

Effects of an Evidence-Based Practice (EBP) Education Program on EBP Practice Readiness and EBP Decision Making in Clinical Nurses

PURPOSE
Today's clinical nurses deal with complex problems that need accurate evidence for practice and decision making. In this study the effectiveness of an EBP education program was examined.
METHODS
A pre-posttest design was used for this study and participants were 46 nurses working at a tertiary hospital located in Suwon, Korea. Date collection was done before and after the education program, from July 27 to October 2, 2015. Data were analyzed using paired t-test and ANCOVA with SPSS 21.0.
RESULTS
There were significant differences in scores before and after the EBP education program for EBP readiness: belief (t=-5.65, p<.001), implementation(t=-2.89, p=.006), competence (t=-4.21, p<.001), and for evidence-based decision making (t=-16.25, p<.001) by the nurses.
CONCLUSION
The findings indicate that the EBP education program has positive effects on EBP belief, implementation, competence and evidence-based decision making. In the future, it is necessary to reinforce the content of the program in the clinical workplace and to provide continuous education for clinical nurses.

PURPOSE
The purpose of this study was to investigate empathy ability, communication, and nursing performance among registered nurses (RN) and nursing assistants (NA) in long-term care hospitals.
METHODS
Participants were 155 nursing personnel (RN 80 and AN 75) who worked in 8 long-term care hospitals in G city and J Province. Data collection was conducted from July 6 to August 6, 2016. Descriptive statistics, independent t-test, one-way ANOVA, and stepwise multiple regression were used with SPSS/WIN program version 23.0 for data analysis.
RESULTS
Nursing performance of RN was significantly influenced by communication ability (β=.88, p<.001) and university graduation (β=.25, p=.025). Nursing performance of NA was influential by communication ability (β=.77, p<.001) and marriage (β=.42, p=.018).
CONCLUSION
Findings show that communication ability of RN and NA is important in nursing performance. Therefore, there is a need to develop programs to improve communication ability and validate the effectiveness of the programs in improving nursing performance of nursing personnel working at long-term care hospitals.

Effects of Workplace Bullying, Job Stress, Self-esteem, and Burnout on the Intention of University Hospital Nurses to Keep Nursing Job

PURPOSE
Medical institutions and their patients benefit from continued employment of nurses. In this study an assessment was done of important factors that influence nurses' intention to leaving their jobs.
METHODS
The sample consisted of 229 university hospital nurses. Data were analyzed using frequency, percentage, mean, standard deviation, t-test, ANOVA, Scheffé, Pearson's Correlation Analysis, and Hierarchical Multiple Regression.
RESULTS
The control variables, including age, current position, and health status explained 18% (F=16.37, p<.001) of variance in retention intention. The control variables, bullying, job stress, self-esteem, and burnout collectively explained 27% of variance in retention intention.
CONCLUSION
The findings indicate that the factors influencing retention intention are age, current position, and health status, while self-esteem and burnout in work places are new and more recent factors that impact retention intention. These findings can be utilized to develop strategies to increase self-esteem and retention intention.

Factors Influencing Nurses' Implementation of Evidence-based Fall Management in Geriatric Hospitals

PURPOSE
The purpose of this study was to identify factors that influence nurses' implementation of evidence-based fall management in geriatric hospitals.
METHODS
Data from the Evidence Based Practice Survey of Fall Management in Geriatric Hospitals were examined for this study. The participants were 248 nurses from geriatric hospitals. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation analysis, and stepwise regression.
RESULTS
Factors related to implementation of evidence-based fall management were belief in evidence-based practice with organizational culture, work experience, use of fall guideline, and experience of patient falls. These factors explained 56.7% of variance in implementation of evidence-based fall management. Among these factors, the significant predictors for implementation of evidence-based fall management were belief in evidence-based practice, organizational culture of evidence-based practice, work experience, use of fall guideline and experience of patient falls.
CONCLUSION
Results of this study suggest that it is necessary to develop a systematic program to enhance nurses' belief in evidence-based practice at the individual level. In addition, fostering organizational culture toward evidence-based practice and disseminating fall management guidelines at the organizational level can be important strategies to enhance evidence-based fall management in geriatric hospitals.

Factors affecting Retention Intention of Male Nurses Working Health Care Institution in Korea

PURPOSE
The study was done to investigate male nurses' gender discrimination, person-organization fit, organization leader-member exchange, career plateau and retention intention and to identify factors affecting nurses' retention intention for these nurses.
METHODS
A cross-sectional survey was conducted using self-report questionnaires. The participants were 144 male nurses working in hospitals and other health care facilities in Korea. Data were analyzed using hierarchial regression.
RESULTS
The mean score was 5.40±1.61 (out of 8) for retention intention. Factors influencing retention intention were ‘married’ (β=.27, p<.001) and ‘employment career (3~5 yr)’ (β=-.24, p=.003) in model 1, ‘person-organization fit’ (β=.42, p<.001) in model 2, ‘content plateau’ (β=-.19, p=.020) in model 3. Person-organization fit was the most significant factor followed by content plateau, employment career and married state in that order. These factors explained 33.2% of the variance in retention intention (F=17.23, p<.001).
CONCLUSION
Findings indicate that it is necessary to confirm that the male nurses are suitable for their organization and improve career development at 3~5 years of work experience as a retention strategy of male nurses.

Nurses' Knowledge of Law, Law Consciousness, and Will to Practice

PURPOSE
The aim of this study was to evaluate nurses' knowledge of the law, consciousness, and will to practice and the relationships among factors affecting the will to practice.
METHODS
A cross-sectional survey was conducted and data were analyzed using descriptive statistics, independent t-test, ANOVA, Pearson correlation coefficients and multiple regression.
RESULTS
Most respondents were women (93.9%) and 75.6% of them had received basic education on law. The average score on knowledge of the law was 11.83±3.07 for general law and 10.42±3.32 for nursing law. The average score on consciousness of law and will to practice was 2.50±0.31 and 4.32±0.58, respectively. Differences were observed in knowledge of the law in terms of having taken a refresher course(F=5.87, p=.003); in consciousness of the law in terms of knowledge of the law (F=6.61, p<.002); and in will to practice according to age (F=7.30, p=.007) and educational level (F=13.08, p<.001). Factors influencing will to practice included behavioral and cognitive consciousness, general knowledge of law, and education. These factors explained 24% of the variance.
CONCLUSION
Nurses knowledge and consciousness regarding law was relatively lower than their will to practice. Systematic law education for nurses and repetitive research are recommended to prevent nursing malpractice.

PURPOSE
The purpose of this study was to identify effects of moving from a functional method of nursing care delivery to a modified team nursing delivery system.
METHODS
A structured self-report questionnaire was used to measure satisfaction with the nursing delivery system, self-efficacy, job stress and nursing performance. Participants were 72 nurses working at a university hospital and data were collected three times; before, one month and three months after the modification to a modified team nursing delivery system. Data were analyzed using repeated measure ANOVA.
RESULTS
Satisfaction with the nursing delivery system increased significantly after the change to the modified team nursing method. There were no significant differences in self-efficacy, job stress, or nursing performance. However, satisfaction with the nursing delivery system and nursing performance increased and job stress decreased significantly in new nurses with less than 1 year experience.
CONCLUSION
Results indicate that a modified team nursing method has positive results on nurses' satisfaction, self-efficacy, job stress, and nursing performance compared to the functional method. Effect of changing the nursing delivery system in the hospital suggests possibilities and directions for a modified nursing delivery system that would be useful in hospitals presently using the functional method.

Policy Issues and New Direction for Comprehensive Nursing Service in the National Health Insurance

PURPOSE
This study was done to identify issues surrounding comprehensive nursing care within the national health insurance, analyze results and problems within national health insurance and to suggest new policy directions for stability.
METHODS
A literature search was performed using RISS, KISS, PUBMED databases. Eighteen studies were analyzed.
RESULTS
For implementation of comprehensive nursing care, there was improvement in nursing and in facilities. By improvement of structure, this new system showed positive outcomes in term of patient satisfaction and indicators related to patient safety. However, issues related to overload, job stress and evaluation of the system remain. In order to establish this system, staffing levels for nurses and nurse aids need to be adjusted to appropriate levels that reflect requirements for nursing. In addition, range of work needs to be determined clearly. Lastly, regular and systematic evaluation is needed to provide safe quality services to patients and to prevent waste of financial resources.
CONCLUSION
Comprehensive nursing care needs to be implemented in ways that patients can be provided with safe and high quality service. There is a need to resolve several issues to allow this new system to function.

Influence of Verbal Abuse on Job Stress for Special Unit Nurses and General Ward Nurses in General Hospitals

PURPOSE
It is expected that there will be a difference in job stress between special unit (operating room, anesthesia room) nurses and general ward nurses in general hospitals, but factors influencing job stress have not been examined. This study reports on research to examine the sources of verbal abuse in the workplace.
METHODS
Participants were 243 nurses (special unit 112, general ward 132) working in a hospital in I city. Data collection period was March 18 to March 25, 2016 and questionnaires were used to collect data. Comparison of differences in influence of verbal abuse on job stress between special unit and general ward nurses was analyzed with multiple hierarchical regression.
RESULTS
In special unit nurses, verbal abuse from doctors and/or nurses influenced their job stress (Adj. R²=.545). In general ward nurses, verbal abuse from patients and/or care-givers influenced their job stress (Adj. R²=.241).
CONCLUSION
Findings indicate a need to campaign for increase respect among nurse-doctor, nurse-nurse, and nurse-patients/care givers because verbal abuse from doctors, nurses, and patients/care-givers causes negative attitudes in nurses. General ward nurses in particular should provide sufficient explanation to patients about nursing services because verbal abuse from patients/care-givers is related to their complaints about nursing care.

PURPOSE
This study was done to identify and compare nursing performance including job stress, emotional labor and burnout between nurses who work in comprehensive nursing care service wards and general wards.
METHODS
Participants were 187 nurses working in a general hospital in Busan which provides comprehensive nursing care service (comprehensive nursing care service ward: 95, general ward: 92). Data were collected using questionnaires and analyzed using descriptive statistics, χ² test, t-test, ANOVA, and multiple regression with the SPSS/WIN 23.0 program.
RESULTS
There was no significant difference in the variables between comprehensive nursing care service ward nurses and general ward nurses. Factors influencing burnout were emotional labor in comprehensive nursing care service ward nurses while job stress and emotional labor were the factors influencing burnout in general ward nurses.
CONCLUSION
The findings of this study indicate that comprehensive nursing care service can be a positive delivery system for nurses. Therefore, there is a need for strategies to integrate comprehensive nursing care service systematically.
